The Village of Hamilton Continuing Care located in Hamilton, Ontario has an exciting opportunity for a Social Services Worker to join their village team on aCONTRACTworking 25 hours biweekly.

As a new Contracted Social Services Team Member joining our village, you will:
Support and embrace our resident-centered social culture and be proud to deliver the highest quality of emotional and personal support to our residents and their family members. They work as key members of our village and neighbourhood multidisciplinary teams to understand residents’ individual needs, and support residents and family members during their transition from home to our Village.
Contracted Social Service Worker at Schlegel Villages foster exemplary relationships with residents, their families and the Village team to deliver holistic care that focusses on flexible living and quality of life. They work closely with our recreation team, chaplain and village leadership to provide group and individual programming, deliver ongoing support during the palliative process and to participate in care planning.
To be successful as our new Social Services Team Member, you will:

Hold a degree is Social Work or Diploma in Social Service Work specializing in Gerontology

Be excited by our objectives, our Village philosophy and the family values that shape all aspects of life in a Schlegel Village

Display a caring and respectful attitude toward our residents, families, and team members
