Detailed Overview

Provides clinical leadership and support to case management staff across the region who work with complex geriatric clients and adults with chronic disease; participates with management and team leaders in the development and implementation of case management practice standards, competencies, clinical education, research, program planning and quality improvement initiatives; provides consultation and support to case managers in client-specific situations.

Responsibilities

Participates in regional and local program planning regarding case management and the needs of complex geriatric population and adults with chronic disease and in the identification of new areas of service delivery based on changing client need. Develops, implements, evaluates and revises education, orientation and professional development programs to staff by conducting learning need assessments of individuals and/or clinical areas by determining educational requirements for staff; introduces new skills and procedures based on current theory, research and standards of care. Acts as a clinical resource and coaches by consulting with staff and clients in complex situations; collaborates with other staff, health care professionals and agencies to plan, implement and evaluate care/support plans. Participates in the process for the resolution of clinical issues and concerns and provides leadership to the development of an interdisciplinary team, integrated care planning and conferencing. Maintains linkages with professional, educational and research resources regarding case management practice, the geriatric population and adults with chronic illness; identifies opportunities for case management staff to maintain and develop competencies and linkages with others regarding case management practice; participates in regional research projects. Participates in the development, system-wide implementation and evaluation of case management practice standards and competencies. Collaborates with team leaders and managers in identifying education needs, in the assessment of the ability to meet standards and competencies, and in the planning, co-ordination, implementation and evaluation of educational programs to support these needs. Participates with clinical leadership and management across the health care system to build relationships and develop and adapt programs that facilitate client's transitions through the system. Sets measurable performance goals and objectives by maintaining and updating own knowledge of clinical expertise within area of practice, developing professional development plans in collaboration with the Manager, Clinical Operations. Performs other related duties as assigned.

Qualifications

Education and Experience

Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Three (3) years' recent related experience in case management, clinical and program development including experience in planning education and facilitating professional development with staff, or an equivalent combination of education, training and experience.

Valid BC Driver's License and access to a personal vehicle for business-related purposes.

Skills And Abilities

Demonstrated organization, communication and team building skills. Demonstrated ability to deal with others effectively. Knowledge of gerontology, geriatrics and the needs of adults living with chronic illness. Specialized knowledge of assessment and planning with clients presenting with complex geriatric needs and adults with chronic disease in the community including neglect and abuse. Knowledge of relevant legislation, standards and policies. Ability to communicate effectively both verbally and in writing. Demonstrated ability to provide leadership. Demonstrated ability to use research in practice. Ability to establish goals and objectives and set priorities and organize workload. Ability to operate related equipment including related software applications. Physical ability to perform the duties of the position.
