About the role

Job Description:

The Behavioral Support Worker is a full-time Registered or non-registered health care professional who will be responsible for providing support and education to the interprofessional team in managing patients that present with responsive behaviors. The Behavioral Support Worker will be accountable to work with the interprofessional team for patient assessment and collaborative care plan development and implementation to address the patient needs. Consistent follow-up and documentation will be integral in this role.

Education:

  • Secondary School Diploma or Equivalent
  • Undergraduate Degree
  • Completion of courses in geriatric care or mental health
  • Gentle Persuasive Approaches in Dementia Care or equivalent training

Experience:

  • 1-3 years of experience in direct care to older adults
  • Recent experience in psychogeriatric care and dealing with responsible behaviours
  • Recent experience in an institutional setting

Competencies:

  • Knowledge of concepts, principles and specific techniques of behaviour management.

Clinical Practice

  • Assessment of patients referred via the interprofessional team. Assessment will be completed by analysis of information obtained through review of the health record and interviewing the patient and family or other health care providers with relevant information for the patients care. Assessment will include a holistic approach
  • Development of an individualized care plan in collaboration with the interprofessional team to assist in managing responsive behaviors. The care
    plan will include measurable and achievable goals with appropriate interventions.
  • Facilitate the implementation of the individualized care plan through communication with patients, families and the interprofessional team. Some interventions will need to be carried out directly by the Behavioral Support Worker (e.g. activation activities, diversional activities, set-up or arrangement of interventions, mobilization of volunteer resources).
  • Document all care planned and/or provided in the patients’ health care record.
  • Monitor effectiveness of the individualized care plan on a routine basis and modify as necessary.
  • Provide just in time education on individualized care plans, patient needs and behavior management techniques and skills to patients, families and RVH employees.
