About the role

Your Opportunity:

The Registered Nurse Specialized in Wound, Ostomy & Continence Care (NSWOC Case Manager (CM) / Interventionist(INT) is responsible for the assessment, planning, implementation and evaluation of care and teaching of individuals/families with complex wound care needs. Reporting to the designated Care Manager within the Consult and Treatment Team (CATT), the NSWOC CM/INT works as a member of an inter-professional Health Care Team in a Wound Clinic environment. The NSWOC CM/INT responsibilities include but is not limited to providing direct client care to HomeCare clients requiring wound care management. The NSWOC CM/INT assesses, plans, implements and evaluates the client care plan; clients may be any age. In addition, the NSWOC CM/INT fosters and promotes continuity of care and co-operative partnerships by collaborating with primary care physicians, Acute care hospitals and HomeCare programs. The NSWOC CM/INT requires close liaison with the client; family members and other health care providers in the community. There is also an expectation to be a mobile remote worker. This role requires travel within the city of Calgary including caring for clients in their homes, clinics or in the community.

Description:

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit holders will be paid $39.58 - $54.16 per hour. Upon obtaining a full licence, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii) as applicable.

Required Qualifications:

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification. Additional Required Qualifications:

Advanced wound care education such as CAWC “S” series; International Interprofessional wound care course (IIWCC), Nurse Specialized in Wound, Ostomy and Continence (NSWOC) course or equivalent (WOC-EP), Skin and Wound Home Care education is required. You will be responsible to operate a laptop computer and mobile phone, and for manual handling (lifting, lowering, carrying, pushing and pulling) as a regular part of the job. For example, A push/pull of up to 50lbs for compression therapy on a regular basis is an expectation of this job. Learning resources and training are provided to safely perform work. Wound Clinic and community positions require and advanced level of skin and wound management knowledge and skills. You will be required to walk and stand on your feet for approximately 75% of your shift and work in the home environments Minimum 2 years wound care experience is required min 3 years experience with Integrated Home Care Excellent interpersonal, communication and time management skills Proven critical thinking, decision making and assessment skills Physically able to perform all assigned tasks Ability to perform all duties in accordance with related legislation

Preferred Qualifications:

Baccalaureate degree in Nursing is preferred. IV qualified is an asset. Basic computer skills an asset.
