As a Placement Facilitator, you will coordinate applications to long-term care homes, as well as facilitate and authorize admissions to long-term care homes (LTCH). These applications can be long-stay, short stay and convalescence care from the community, hospitals and other long-term care homes. You will act as an information resource for internal and external stakeholders.
Whether you work in our office, in the community, or a health care facility – you will play a lead role in providing connected, accessible, patient-centered care – and be supported by our collaborative team that includes over 8,000 regulated health care and other professionals.

What will you do?
Receive and monitor information regarding vacancies in long-term care homes within the South East geography Manage the wait-lists for Ontario Health atHome in the area of South East long term care homes Negotiate with Ontario Health atHome South East LTCHs around appropriate applicants for the available vacancies. Contact the patient, power of attorney (POA) or substitute decision maker (SDM) to conduct a bed offer for available beds and advise the patient/POA/SDM of the financial costs Coordinate LTCH Admissions Advise Completes “Authorization for Admission” form for the Ministry of Health and Ministry of Long Term Care Documents admissions according to policies and procedures Educates and liaises with applicant/POA/SDM in order to achieve a successful LTC admission. Ensures that information regarding vacancies is kept current on the data base Closes files on an appropriate and timely basis.
What must you have?
Registered Practical Nurse (RPN) Current registration with the College of Nurses of Ontario as an RPN in the general class Health Care experience in LTC or equivalent Ontario Health atHome in the area of South East experience is preferred Knowledge of Long Term Care Act, Substitute Decision Act, and Health Care Consent Act Strong data entry skills and ability to work with MS Word, Outlook, Excel A valid driver’s license and access to a reliable vehicle Proficient in a Windows environment We have a mandatory COVID-19 vaccination policy. As a condition of employment, all employees are required to submit proof of COVID-19 vaccination status prior to start date.
