POSITION OBJECTIVE:
As a key member of the interdisciplinary health care team, you will apply your critical thinking skills and knowledge of drug therapy in the provision of safe and effective medication-related care for patients of Niagara Health. Responsibilities will include, but are not limited to, addressing patient-specific drug related problems, collaborating with prescribers in the optimization of medications, clinical review and processing of prescriber orders, supporting the Medication Reconciliation process, engaging in various quality improvement/safety initiatives or projects. The pharmacy program is committed to the learning organization culture. As such, you will act as a preceptor to both pharmacist students and interns. You will work collaboratively with other health professionals, meeting your scope of practice within the hospital environment that is guided by policy and the pharmacy practice model of the organization.
QUALIFICATIONS:
o BSc Pharmacy or PharmD degree
o Current registration with the Ontario College of Pharmacists in part A of the register
o Hospital pharmacy residency required; or related clinical experience considered
o Demonstrated ability to prioritize workflow and patient care activities
o Flexible, self-directed and able to work effectively with minimal supervision
o Demonstrated effective problem solving and critical thinking skills
o Demonstrated proficient computer skills
o Previous experience with the Pharmacy Meditech module preferred
o Evidence of professional development and continuing education. Excellent communication and interpersonal skills
o Demonstrated leadership qualities
o Demonstrated ability to communicate and work effectively within a multi-disciplinary team
o Consistent and regular attendance required
o Proficiency in both official languages (English & French) will be considered an asset
