About the role
Spring Valley Care Center is looking for an experienced social worker to join our team in our facility in Rutland, Kelowna. This person will work to support the daily operations of the office in a part time capacity, 10-15 hours per week at $55-$65 per hour. This is a contracted hourly role and does not include benefits.
The ideal candidate is a high-energy individual with excellent organizational skills, a strong attention to detail and the ability to communicate effectively in both written and verbal communications and has experience working with Ministry, PG&T, Bloom Group and other organizations to assist residents and families in securing funding for Long Term Care placement.
JOB SUMMARY:
The Social Worker holds a leadership position in the Park Place Home and takes a lead with the
interdisciplinary team through all Resident Move-In processes while also providing guidance and
comfort to new Residents and their families. Provides Residents and their families with professional
Social Work services by developing, implementing, and monitoring programs which meet the social,
emotional, spiritual, and intellectual needs of the Residents. Is the lead for the ethics program, and
annual surveys (family, Resident, and employees). Ensures a commitment to Resident safety and to
our “culture of safety” as per the Strategic Directions of Park Place Seniors Living.
TYPICAL DUTIES AND RESPONSIBILITIES:
- In collaboration with the clinical leadership, coordinates Move-In/Move-Out of Residents,
through evaluation and assessment of each individual with respect to appropriateness of
placement, ability of the Park Place Home to meet his/her identified needs, and the needs of
the community. - In collaboration with the clinical leadership, responsible for Pathways System and/or
communication with health services’ Transition Services. - Provides transitional support to family and Residents upon Move-In, documenting as per Park
Place policies. - Participates in Move-Out planning, and provides follow-up after moving out, as necessary.
- Promotes Resident autonomy in decision-making for health care, personal, and financial
matters. - In collaboration with the Site Leader, assists Resident and family members to source resources
in the areas of substitute decision-making, guardianship, trusteeship, Personal Directives and/or
Enduring Power of Attorney. - In collaboration with the Site Leader, assists Residents and families in utilizing financial, legal,
mental health and other community resources to enhance the quality of Resident life. - Contributes to the development, implementation, and evaluation of the Resident care plan as
an interdisciplinary team member. Advocates on behalf of the Resident and/or family as
appropriate - Participates in care conferences. Assists in resolving family issues and complaints if
appropriate. - Completes required documentation (i.e. social history, assessment, progress notes) in a timely
manner. - Attends and participates in appropriate committees and meetings (e.g. Leadership Team, End
of Life Committee). - Attends applicable in-services and participates in orientation, in-service training and counseling
of employees on a request basis. - Takes the lead in the Park Place Home’s quality improvement/performance measurement
initiatives and processes including follow-up with summary and action plan implementation.. - In collaboration with Recreation Department plans and conducts Memorial Services
- Performs other duties as required.
- Coordinates and participates in MDS data entry by communicating and observing the Resident,
communicating with direct care staff, interdisciplinary health professionals, and family. - The Social Worker is responsible for both the Minimum Data Set (MDS) and the Resident
Assessment Protocols (RAPs) as it pertains to Move-Ins, Move-Outs, transfers, and Social
Work scope of practice.
HEALTH AND SAFETY RESPONSIBILITIES:
- Responsible to read, understand and comply with Park Place's and provincially mandated
Occupational Health and Safety policies and safe work practices.
- Involved in all aspects of the Health and Safety Program including:
o Uses safe work procedures,
o Promotes health and safety policy and awareness,
o Makes safety suggestions.
o Reads OHS minutes posted.
- Responsible to take every reasonable precaution to protect the safety of Residents, self, other
employees, and the public.
- Reports any near miss, injury, accident or equipment damage to supervisor immediately and
completes required reports.

Corrects and/or reports unsafe conditions.

Sets a good example.
QUALIFICATIONS AND EXPERIENCE:
Baccalaureate Degree in Social Work or its equivalent from an accredited School of Social Work
preferred.
Recent related social work experience or experience working with the elderly is preferred.
