About the role

St. Joseph's Health Centre, Unity Health Toronto, is proud to announce the opening of a new 21-bed Reactivation Care Unit located on 4G. This unit is dedicated to providing collaborative care for adult and geriatric patients who require therapy, reactivation, and transition planning to successfully transfer to their community or other supportive environments.

The Reactivation Care Unit focuses on patients who no longer need acute care but still require restorative care to aid their transition home or to other settings. As a member of our dedicated interprofessional team, you will deliver patient-centered care based on best practice principles. Our team will work closely with patients, families, and caregivers to provide reactivation therapy aimed at enhancing and optimizing patients' functional abilities and engagement in daily routines.

Job Profile:

Under the direct supervision of the Registered Nurse/Registered Practical Nurse (RN/RPN):

  • Provides hands on care including bathing or washing, toileting/incontinence care, grooming (i.e. hair care, skin and nail care), oral hygiene, dressing, transferring;
  • Observes each patient, consults with and reports any issues or changes with each individual patient to the RN/RPN;
  • Contacts the RN/RPN when the patient's needs are beyond the limits of the Health Care Attendant role;
  • Follows the care plan instructions for bowel and bladder care and routines;
  • Assists with personal hygiene maintaining the infection control standards;
  • Utilize assistive, safety and rehabilitative devices including hearing aids, mechanical lifts, walkers, wheelchairs, restraints as per care plan;
  • Assists the RN/RPN with the admission process and ongoing observation as well as providing input into the development of a care plan;
  • Supervising groups during meal time;
  • Transporting patients as required;
  • Participates in educational programs / in-services as required;
  • Contributes information to the care planning meetings regarding patient /family needs;
  • Adheres to Occupational Health and Safety Legislation and Unity Health Toronto Health and Safety Policies (i.e. Lift and Transfer Policies);
  • Adheres to Unity Health Toronto Policies and Procedures, including Incident Reporting Policy.

Qualifications:

  • Personal Support Worker (PSW) certificate from an accredited community college/approved by the Ontario Ministry of Colleges and Universities that is no less than 625 hours in length
  • Current certification in Basic Lifesaver, Basic Rescuer C or Heartsaver Plus.
  • Recent acute care experience.
  • Demonstrates excellent interpersonal and communication skills, both verbal and written in the English Language.
  • Ability to perform FULL range of duties in this position including specific types of patient care as directed within the list approved for the category and by the Regulated Health Professions Act (RHPA).
  • Ability to provide porter and transportation services for patients and supplies.
  • Ability to rotate on day, evening and night shifts as scheduled.
  • Satisfactory attendance record.
