The Carefirst Family Health Team (FHT) is seeking Full-time permanent Registered Dietitian to join our innovative, interdisciplinary team dedicated to delivering high-quality, community-based care.
About Us
Carefirst FHT is committed to delivering exceptional, comprehensive primary care through a collaborative, team-based model that includes family physicians, nurse practitioners, nurses, social workers, dieticians, physiotherapists, and medical receptionists. We have a robust Chronic Disease Management and Prevention program, including a Diabetes Education Program, and are actively involved in many of the Ontario Health Team’s primary care initiatives.
Our strong community partnerships ensure coordinated, high-quality, patient‑centred care across the continuum. We work closely with the Carefirst Seniors & Community Services Association to provide truly integrated services.
On‑site & Integrated Programs/Services include:
- Ontario Health atHome (formerly Home and Community Care Support Services)
- MINT Memory Clinic
- Ontario Structured Psychotherapy (OSP) Program
- Specialist clinics: psychiatry, cardiology, endocrinology, nephrology, optometry, geriatric medicine, gynecology, chiropody, and audiology
Clinic Locations
We currently operate four well-established clinics in Scarborough, Markham and Richmond Hill. Beginning in Fall 2025, we will be expanding in Markham and Stouffville to support continued growth and community needs.
Employees will be assigned to a location that aligns with their preferences and residence, with occasional rotation to support clinic operations.
We are currently seeking a Registered Dietitian to fill a new position at our Family Health Team.
Terms: Full Time Permanent
Pay Range : $65,000 - $75,000 annually
Responsibilities:
- Work with individual patients to determine nutritional needs
- Assess patient nutritional status by gaining an understanding of food habits or preferences, socio-economic and cultural background, psychological factors and medical profile
- Conduct nutritional and anthropometry assessment
- Develop, implement and evaluate individualized nutritional care plans for patients, based on comprehensive needs assessment i.e., considering literacy, language, cultural and psycho-social factors
- Provide nutritional advice and counselling to patients and work with multidisciplinary team and develop client-centred care plans for patients
- Provide Diabetes Education and co-design diabetes self-management plan with patients
- Ensuring practice is consistent with professional standards and the ethical framework as outlined by the College of Dietitians of Ontario and organizational policy, practice and values
- Demonstrating commitment to professional life long learning by participating in continuing education activities to meet the needs of the patient population served and the standards of the profession
- Performing reflective practice, analyzing and evaluating dietetic knowledge and modifying practice accordingly to improve the quality of patient care
- Performing cross-functional and other responsibilities, as assigned and/or requested
- Provide support to Chronic Disease Prevention and Maintenance Program through direct service delivery including outreach, health education, nutritional counselling and others
- Manage statistic record of diabetes program services, and prepare quarterly reports
- Perform other duties as assigned
Requirements:
- Bachelor’s degree of science in Nutrition or equivalent
- Completion of Dietitians of Canada Accredited Internship or Practicum Programs or equivalent (preferably with a community focus or equivalent approved experience)
- Registration and in good standing with the College of Dietitian of Ontario
- Eligibility for a membership in the College of Dietitians of Ontario and Dietitians of Canada.
- Certified Diabetes Educator (CDE) certification preferred
- At least three years relevant experience or At least two years work experience as a Registered Dietitian in a community based health service organization
- Knowledge of and sensitivity to the impact of social, economic, environmental and cultural issues in diet and nutrition
- Excellent interpersonal and communication skills (i.e. verbal and written)
- Good organizational and time management skills
- Excellent problem-solving and patient service skills
- Experience in planning and implementing health promotion programs and activities
- Familiarity with client-centered and self-management approaches to chronic disease prevention and management would be an asset
- Valid CPR/first aid certification
- Proficiency in English and a second language reflecting the community served, such as Hindi, Tamil, Urdu is an asset
- Available to work flexible hours including some evenings and weekends
- Current vulnerable sector check (completed within one year)

Carefirst Seniors & Community Services Association is a non-profit community services agency. Since 1976, we have provided community support, home and personal health care services in a holistic manner to seniors, individuals with disabilities, and those with medical needs in the Greater Toronto, and York regions.
With a staff of 500 members and the help of a pool of 1,200 active volunteers, we serve over 7,000 clients a year, including seniors and the physically disabled; 1,500 of whom are "home-bound" and frail.
