ABOUT TRAVERSE INDEPENDENCE
Traverse Independence was founded in 1975 as an agency dedicated to developing housing with support services for people with physical disabilities. Since then, our organization has evolved with a new name, as well as from offering services for adults with physical disabilities to serving those with a brain injury through eight facilities in the Region of Waterloo and County of Wellington. We are dedicated to the mission of maximizing our client's ability to live independently by providing support services for adults with physical disabilities or brain injuries.
A DAY IN THE LIFE OF A LIFE SKILLS FACILITATOR
As aLife Skills Facilitator, you rely on your strong previousexperienceto learn the ins and outs of all your essential daily duties. This position is based out of our day program site, which provides services to adults with acquired brain injuries. Typical duties at the day program site include, but are not limited to: implementation of behaviour management techniques to ensure life skills programs are delivered effectively, teaching and retraining lost or impacted skills based on the analysis of client requirements (using very specific teaching and monitoring models), hands-on provision of attendant care and life skills programs, and you will work collaboratively with the Traverse Independence team, the client, his/her caregivers, family members, and other service providers to ensure the highest quality of services are delivered. You willcommunicateeffectively with our clients and their families to ensure that they have the best quality of care. In addition, you will leverage your computer skills to keep up with charting and updating all records. This job requires you to have aflexible scheduleand reliable transportation.
QUALIFICATIONS
- Possess a Community College Diploma in Social Services, Health Care or related discipline
- 3-years experience providing personal care and working with complex clientele (experience supporting clients with disabilities is an asset)
- Experience in behaviour management skills
- Excellent organizational skills, interpersonal skills and communication skills (both written and oral)
- Proven ability to work as a member of a multi-disciplinary team
- Demonstrated computer proficiency and capacity to adapt to new software/systems
- Existing knowledge of community resources is an asset
- Valid Level C CPR/Standard First Aid Certification
- Ability to pass a background check
- Reliable vehicle
- Valid class G driver's license in good standing
- Proof of $1,000,000 liability insurance
- Current drivers abstract required (acceptable to Traverse Independence)
