Applicants must reside and be legally eligible to work in Ontario. This is a Regular Full-Time Opportunity Type: New An exciting opportunity as a Regular Full-Time, Registered Practical Nurse (RPN) exists in the Women’s College Academic Family Health Team (WCAFHT), reporting to the Executive Director, Environmental Health Clinic and WCAFHT. The Women’s College Academic Family Health Team is one of the largest Family Practice Units within the Department of Family and Community Medicine with over 40 staff Physicians, interprofessional team, administrative and support staff.

Summary Of Duties, But Not Limited To

  • Responsible for providing nursing care, collaborating with other health professionals and participating in the organization of activities in the provision of patient care
  • Practices within his/her full scope of practice as outlined in the Standards of Practice by the College of Nurses of Ontario.
  • Liaises with Physicians, Nurse Practitioners, Socials Workers, and other team members to ensure team and unit functions efficiently and provides continuity of patient care
  • Assesses patient phone calls and responds to patient concerns in accordance with the standards established by the College of Nurses of Ontario
  • Assist patients in accessing appropriate health care resources in the community through advocacy and follow up care
  • Records patient information in a precise, concise and organized manner using an electronic medical record
  • Ensures patients receive appropriate standards of care and follow-up which may involve chart review, recalling patients for appointments etc.
  • Organizes necessary follow-up arrangements/referrals to community-based services for patients.
  • Supports patient intake and coordination.
  • Works with a diverse and complex population and community partners to enhance patent care and access to resources.
  • Works collaboratively with the interprofessional team at full scope of practice to enhance team based primary care.
  • Incorporate a wide knowledge base to facilitate professional responsibilities in the provision of patient care including patient education on topics such as heart health, diet and exercise

Qualifications

  • Registered Practical Nurse Diploma
  • Current Certificate of Competence (RPN) from the College of Nurses of Ontario.
  • Basic Cardiac Life Support certificate
  • A minimum of 3-5 years’ experience in a Primary Care/Family Practice setting providing full scope of care to a primary care patient population.
  • 2 years’ experience with Electronic Medical Records (ERM), preferably TELUS/PSS and EPIC
  • Excellent computer skills and a minimum of 2 years’ experience with an Electronic Medical Records (EMR) Primary Care system or equivalent experience required.
  • Successful completion of a Nursing Physical Assessment Course, Solution Focused Course, and Pharmacology Course, an asset
  • Demonstrated experience working with diverse and complex patient population.
  • Demonstrated experience with patient intake and \ referral coordination.
  • Ability to work in a fast-paced team environment.
  • Excellent patient assessment, triaging skills, and teaching experience
  • Demonstrated effective interpersonal and communication skills Ability to organize time and set priorities effectively.
  • Ability to work independently with minimal supervision.
  • Professional behavior and communication that meets the standards of the professional regulatory college or association, as applicable, and the standards of Women’s College Hospital
  • This position plays a critical role in acting as an advocate for safety and will demonstrate principles, practices and processes that will optimize a safe environment for all
