Social Worker - Housing (12 - 18 Month Contract)
About the role
Key Responsibilities
- Develop individualized housing and service plans, provide crisis intervention, and offer consistent case management.
- Assist clients in navigating subsidized and affordable housing options, completing applications, and understanding tenancy rights and responsibilities.
- Conduct home and community visits to assess housing conditions, identify risks, and provide ongoing support.
- Advocate for client access to housing services, subsidies, and community supports by collaborating with housing providers, government agencies, and partner organizations.
- Build and maintain strong relationships with external service providers, community partners, and internal teams to support holistic care.
- Refer clients to appropriate in-house supports (e.g., mental health, employment) and external resources to meet broader needs.
- Maintain accurate and timely documentation in accordance with agency policies and privacy legislation.
- Adhere to all applicable legislation, regulations, and standards of the Ministry and JF&CS.
- Perform other related duties as assigned.
What You Will Bring:
- Bachelor of Social Work (BSW) or equivalent education
- Registered and in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
- 2–3 years of clinical or related experience, preferably in housing or homelessness services
- Strong skills in assessment, case management, and client-centered service delivery
- Experience working within anti-oppressive, trauma-informed, and Housing First frameworks
- Knowledge of housing systems, social services, and community resources
- Comprehension of French or another language will be considered an asset
