Social Worker - Housing (12 - 18 Month Contract)

About the role

Key Responsibilities

  • Develop individualized housing and service plans, provide crisis intervention, and offer consistent case management.
  • Assist clients in navigating subsidized and affordable housing options, completing applications, and understanding tenancy rights and responsibilities.
  • Conduct home and community visits to assess housing conditions, identify risks, and provide ongoing support.
  • Advocate for client access to housing services, subsidies, and community supports by collaborating with housing providers, government agencies, and partner organizations.
  • Build and maintain strong relationships with external service providers, community partners, and internal teams to support holistic care.
  • Refer clients to appropriate in-house supports (e.g., mental health, employment) and external resources to meet broader needs.
  • Maintain accurate and timely documentation in accordance with agency policies and privacy legislation.
  • Adhere to all applicable legislation, regulations, and standards of the Ministry and JF&CS.
  • Perform other related duties as assigned.

What You Will Bring:

  • Bachelor of Social Work (BSW) or equivalent education
  • Registered and in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
  • 2–3 years of clinical or related experience, preferably in housing or homelessness services
  • Strong skills in assessment, case management, and client-centered service delivery
  • Experience working within anti-oppressive, trauma-informed, and Housing First frameworks
  • Knowledge of housing systems, social services, and community resources
  • Comprehension of French or another language will be considered an asset
