Summary of Position

The Community Engagement Coordinator reports to the Director and is integral to the operations of the Innovative Partnerships portfolio. The coordinator is responsible for the day-to-day coordination and efficient delivery of initiatives and partnerships. The job also involves frequent communication with stakeholders to facilitate meaningful community partnerships. The key purpose of this role is to effectively support new initiatives, coordinate and support current programming within project and partners portfolio and steward partnerships with community agencies to help deliver high-quality care for patients.

Key Responsibilities

  • Coordinate meetings with stakeholders to encourage collaboration, implementation and maintenance of projects
  • Build and maintain relationships with stakeholders, including community partners, physicians and health team members
  • Facilitate collaborative meetings to ensure all perspectives are heard and valued
  • Manage patient and case referrals for case collaborative tables
  • Coordinate all aspects of case collaboratives, including meeting scheduling, EMR documentation, care planning and follow‑up support
  • Manage intake processes for project management and patient advisory groups
  • Contribute to the design, implementation and maintenance of health projects in collaboration with community partners
  • Develop documents and tools to support projects and operational processes
  • Conduct environmental scans of services and resources available for patients
  • Assist with writing informational content for patients, physicians and the health team regarding specific health topics in collaboration with Communications
  • Research evidence-based practices and assist in writing literature reviews
  • Contribute to project plans in collaboration with the project team, including monitoring and reporting on milestones
  • Assist Program Managers with programming and facilitation of community partnership groups, advisory groups and patient experience
  • Contribute to evaluation planning and data collection in collaboration with the Evaluation team
  • Represent the PCN at community events, including interagency meetings, community fairs and forums to promote our programs and services and seek out opportunities for collaboration
  • Serve as a point of contact for community partner inquiries
  • Scope out partnership opportunities
  • Assist Program Managers in finding and acquiring outside funding sources for key initiatives
  • Coordinate training and learning opportunities for stakeholders
  • Support change management activities, including stakeholder engagement in projects and partnership initiatives

Qualifications

  • Post-secondary education: Diploma or bachelor’s degree in Project Management, Community Development, Social Sciences, Nursing, Business or other related field. Equivalence of education and experience may be considered
  • Minimum three to five years of community engagement experience
  • Must possess a driver’s license and have daily access to a reliable vehicle
  • Experience in healthcare field
  • Facilitation training and experience
  • Experience with committees and working groups
  • Knowledge of change management frameworks and principles
  • Community development experience
  • Adept in use of EMR preferred
  • Experience working with digital collaboration tools to support project coordination, including project management platforms and shared collaboration environments such as Microsoft Teams, Planner, SharePoint, Power BI or similar

Employment Requirements

  • Completion of a satisfactory criminal record check and/or Vulnerable Sector Search.
