What you will do:

  • Deliver patient-centred care through individualized assessment, treatment planning, and ongoing evaluation of progress.
  • Coordinate rehabilitation services, including regular RC sessions, scheduling, and treatment plan updates.
  • Liaise with clients, families, referral sources, funders, and internal teams to support continuity of care.
  • Contribute to team excellence through collaboration, sound judgement, integrity, and continuous professional development.

What you will need to be successful:

  • You have a Bachelor of Social Work or a Master of Social Work.
  • You are a registered member in good standing with the Alberta College of Social Workers.
  • 1 to 2 years of community experience is an asset.
  • You are passionate, empathetic, use good judgement and apply critical thinking when making client decisions.
  • You are known for your excellent communication skills and ability to work in collaboration within an interdisciplinary team as well as work independently.
  • You have ability to travel within the Hawkesbury community as part of the CBI Health Home Health Services team.
