3 – 1.0 FTE Temporary Contract (8 weeks)

One of a network of provincial community health centres, the NorWest Community Health Centres is a community based, non-profit organization funded by Ontario Health, emphasizing access to primary health care and prevention programs and services.
We are currently seeking three (3) FTE temporary contract Community Health Workers, who, as valuable members of the Community Health Centre team, will provide support to clients to improve health and well-being, reduce social isolation through program delivery, and connect clients with long-term community services. The Community Health Worker works closely with the Allied Health Programs, inter-professional care team and community partners to support clients in meeting their needs and achieving goals related to the determinants of health.
The Health Care of Ontario Pension Plan (HOOPP) is available as an optional benefit.
The rate of this position is $17.60 per hour. We do not use AI to screen resumes.
QUALIFICATIONS:
- Currently enrolled as a student and interest in social work or community health services
- Ability to support clients given a social prescription for tasks
- Ability to function as part of a multidisciplinary team in a community health environment.
- Ability to organize opportunities for community members.
- Promote opportunities for volunteers and community members.
- Must be between 15 and 30 years of age at the beginning of the employment period
- Canadian citizen, permanent resident, or person to whom refugee protection has been conferred under the Immigration and Refugee Protection Act for the duration of the employment** and
- Have a valid Social Insurance Number at the start of employment and be legally entitled to work in Canada in accordance with relevant provincial or territorial legislation and regulation
