About the role

Your Opportunity:

If you enjoy a role in providing services to clients in a community based setting then this position will be of interest to you. The Licensed Practical Nurse (LPN) in Home Care works as a member of an inter professional health care team utilizing the nursing process to provide direct nursing care as well as provide case management to individuals across the life span. The LPN is responsible for their nursing practice as defined by the College of Licensed Practical Nurses of Alberta (CLPNA). In collaboration with the Registered Nurse Case Home Care Manager, the LPN is responsible to assess, plan, implement, case manage and evaluate the needs of Home Care clients and their caregivers/families. There is also an expectation to be a mobile worker. This role requires travel within the Chestermere and sometimes the Strathmore area including caring for clients in their homes or in the community. Access to a motor vehicle in good repair for work related purposes is a requirement of this position. You will be responsible to carry and move equipment, use and operate a laptop computer and mobile phone, and for manual handling (lifting, lowering, carrying, pushing and pulling) as a regular part of the job. Learning resources and training are provided to safely perform work.

You will be guided by your education, experience, and demonstrated skill level, in alignment with applicable legislation, standards of practice, policies, and procedures. Required Qualifications:

Completion of an accredited practical nurse education program. Active or eligible for registration and a practice permit as a Licensed Practical Nurse with the College of LPNs and HCAs of Alberta (CLHA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).

Additional Required Qualifications:

Two years of recent home care or acute care experience in the last five years Advanced wound care knowledge and experience required Must be proficient in computer use as role is heavily dependent on computer use Knowledge, skill ability and willingness to work to full scope of practice Ability to use critical thinking skills Critical inquiry to assess, plan & evaluate

Preferred Qualifications:

Experience working with acute clients and/or long-term clients with a focus on chronic disease management Experience in providing case management services Experience in Home Parenteral Therapy (HPT), wound, skin and ostomy care, palliative support and client teaching
