Position Summary :

The clinical pharmacist will join the specialized PREVENT clinic team, a multidisciplinary cardiovascular risk reduction program dedicated to preventing heart attacks and strokes. This role focuses on comprehensive patient consultations, cardiovascular risk assessment, and expert management of complex hypertension and heart failure cases.

The pharmacist's primary objective is to provide direct, responsible pharmaceutical care to improve patients' quality of life through evidence-based medication optimization. Working within a patient and family-centered care model, the pharmacist ensures that individual health needs and desired outcomes drive all clinical decisions and quality measurements.

The pharmacist will collaborate closely with patients, families, and the interdisciplinary health care team to optimize patient care. Additional responsibilities include educating patients and healthcare staff about medications, mentoring pharmacy learners, and ensuring safe, efficient medication management within the institution, including controlled substances and hazardous medications.

POSITION REQUIREMENTS

· Applicants must hold entry to practice Pharmacy degree from an accredited University (e.g. PharmD, BScPharm)

· Must hold licensure with the Ontario College of Pharmacists and be in good standing

· Successfully able to perform their function in accordance with prescribed standards that focuses on the priority of People Safety and the delivery of the highest standard of Quality Care.

· Demonstrate the physical and mental capabilities necessary for proper and efficient performance of the essential duties of the position.

PREFERRED REQUIREMENTS

· Completion of an accredited Canadian Pharmacy residency (or equivalent) is preferred

· 3+ years of relevant clinical practice experience, preferably in an ambulatory care setting

· Completion of specialized cardiovascular pharmacotherapy training or demonstrated expertise in hypertension and heart failure management

· Experience with ambulatory blood pressure monitoring and interpretation

· Familiarity with cardiovascular risk assessment tools and shared decision-making approaches

· Demonstrated experience in research project design, implementation, and analysis is preferred, particularly where outcomes have informed policy, clinical practice, or operational improvements.

· Experience in quality improvement methodologies and outcome measurement
