Position Summary

Reporting to the Manager of Pharmacy Services, you will work collaboratively with the pharmacy team, clinical teams and physician partners to provide safe and accurate medication management and pharmaceutical care services

Duties include, but not limited to, medication order entry into Meditech, attending clinical rounds, medication profile review, work with prescribers to identify and resolve any drug therapy issues, act as a resource for medication related questions for prescribers, clinical teams (e.g. nurses, allied health) and pharmacy technicians, Identify and follow up on adverse reactions and medication incidents, maintain JBH confidentiality and privacy standards at all times, follow the Pharmacist Standards of Practice as outlined by the Ontario College of Pharmacists, and other duties as assigned by the Manager

Qualifications:

  • Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD) from an accredited university (required)
  • Registration with the Ontario College of Pharmacists, Part A, in good standing (required)
  • Hospital pharmacy residency preferred
  • Meditech computer experience an asset
  • Previous hospital experience an asset
  • Patient centred with attention to details
  • Excellent problem solving skills
  • Proficient in the use of Microsoft Office programs
  • Ability to multitask and work well under pressure
  • Committed to service excellence and patient safety
  • Excellent verbal and written communication skills

Professional Affiliations:

  • Ontario College of Pharmacists (mandatory).
  • Canadian Society of Hospital Pharmacists (preferred)
  • Ontario Pharmacists’ Association (preferred).
