Role Level: CUPE09 ($35.807 - $38.161)

Shifts : All shifts | Monday - Sunday | Mix of 4.0 hrs, 6.0 hrs, 7.5 hrs and 11.25 hrs

Responsibilities:

  • Assist home hemodialysis and Peritoneal patients with treatments within the home setting according to treatment plan while prompting quality of life for patients.
  • Must complete and maintain related records and documentation.
  • Must continuously observe patients and assist with necessary machine alarms; and must report to the primary registered nurse status changes and document.
  • Assist teams with necessary supplies to provide dialysis treatments.
  • Responsible for correct use of equipment according to guidelines and protocols; and reports any piece of equipment that is out of order and remove it from use.
  • Follows all infection control precautions and protocols.
  • Reports any requests or complaints made by the patients; and families to the Primary Home nurse or Home Transition Unit Coordinating Nurse (CN) or Peritoneal dialysis coordinating nurse
  • Maintains confidentiality in all aspects of work.

Qualifications:

THP is committed to internal talent, and therefore priority will be given to internal applicants.

  • Candidate will be a diploma graduate of a recognized college or school of nursing or equivalent (i.e., certificate RPN with added courses in medication administration, and health assessment)
  • Current certification or eligibility for certification of competence from the College of Nurses of Ontario is required.
  • Current BCLS
  • Minimum of two years recent dialysis or Medical-surgical experience required in an acute care setting.
  • Knowledge of the Nephrology Nursing Standards and Practice Recommendations and Canadian Association of Nephrology Nurses and Technologists (CANNT) as they relate to hemodialysis including all procedures pertaining to patient preparation, treatment, and monitoring during and after procedure required.
  • A physical assessment course required.
  • Demonstrates knowledge of care for the patient with end stage renal disease
  • Recent experience with the renal patient population preferred.
  • Demonstrates knowledge of and experience with the following: wound, skin, and foot care; the diabetic patient; venipuncture; routine practices of infection control; normal and abnormal blood values
  • Demonstrates technical ability.
  • Previous experience in patient education programs
  • Demonstrates excellence in communication skills and teamwork.
  • Demonstrates strong attention to patient and family-centred care and establishing therapeutic relationships.
  • Evidence of ongoing professional development preferred.
  • Assignments will include Community Home dialysis, Renal Care Centre satellite unit, Speakman dialysis care unit, CVH and Home Transition Unit
  • A positive work and attendance record is essential.
  • Valid Drivers licence in good standing with reliable vehicle to travel within the THP ‘s community.
  • Must be self -directed individual with well developed organization skills.
  • Posses the ability to take initiatives and work within minimal supervision.
  • Travelling required
