What the Program Entails

SWCHC is well known for its excellent Community Health Centre (CHC) model and its professional health services team, but it has also become a leader in offering a wide range of community-building activities and advocacy programs. Our team of doctors, nurses, dietitians and more offer a range of medical services.

What You’ll Do

Reporting to the Director of Family and Community Services, the Clinic Nurse works as part of an interdisciplinary team to provide coordinated care within FCS, the Primary Health Care Clinic, and Homelessness and Addiction Recovery Treatment (HART) Hub. The Clinic Nurse is responsible for delivering healthcare services to Black and racialized populations with complex chronic diseases. The Clinic Nurse will play an active role in health promotion during wellness and mobile clinics, conduct community outreach events in collaboration with internal SWCHC teams and external partners, and support the evaluation of our health programs to this diverse population. Functioning within a robust team and collaborative environment, the Clinic Nurse will help advance program goals and enhance health outcomes within the community.

What We’re Looking For

  • Life and Work Experience

  • Three to five years’ experience in community nursing working with diverse populations

  • Previous clinic experience an asset

  • Previous experience using an EMR, preferably Practice Solutions Suite (PSS)

  • Experience providing routine immunizations for all ages

  • Your Education Path

  • Current license with the College of Nurses of Ontario (RN/RPN)\

  • Current CPR (preferred)

  • Required Language

  • Fluency in English required

  • Working knowledge of French or other languages an asset

What You Bring to the Table

  • Knowledge of primary health care and the social determinants of health
  • Able to work effectively in an inter-professional environment
  • Knowledge and awareness related to cultural competence
  • Experience working with disadvantaged communities
  • Knowledge of nursing care related to Primary Care/Community Health/Chronic Disease Management

As our Clinic Nurse, your responsibilities include, but are not limited to:

  • Nursing assessments
  • Telephone & in-person triage
  • Administer medications & treatments
  • Client education
  • Perform procedures within the scope of practice such as wound care, ear syringing
  • Client advocacy
