Horizon Health Network (Horizon) is seeking a strategic, passionate, driven individual to join our team as a Clinical Liaison – Capital Project Manager. The ideal candidate will have experience leading projects using a formalized methodology that is aligned to industry best practices, and a proven track record of delivering results that demonstrates a keen ability to see projects through from conception to conclusion.

Who You Are
You have worked in healthcare or other industry managing complex projects of various sizes for a minimum of 5 years. You have experience in project management and leading multidisciplinary teams, as well as understand continuous improvement concepts and change management. You have a proficiency level in Microsoft Office applications. You have strong analytical, organizational problem solving and communication skills. Experience coaching and/or mentoring would be considered an asset.
Education Requirements
University degree or an equivalent combination of education/experience in formal project management Preference will be given to candidates with PMP certification
What You Will Do
Coordinate and manage projects with multi-disciplinary teams of health care professionals, support staff, executive leaders, and various other stakeholders, including external partners and vendors. Oversee all planning activities, including development of the project charter, requirements gathering, scope definition, schedule development, human resource planning, cost estimating, risk planning, stakeholder analysis, communications and change management planning. Take an active role in managing project relationships. Work with project sponsors, business/process owners, and other stakeholders to foster productive and effective collaboration across the project organization, determining stakeholders needs, and managing expectations. Execute the project plan, ensuring the project remains within its schedule, cost, and scope baselines, performing change control procedures when required. Collaborate with Program teams/Transitional Planning Manager in the planning, review, and coordination of furniture and equipment (F&E) requirements to coordinate move logistics, minimizing disruptions to patient care and operations Monitor and report on project progress, manage risks and issues through project life cycle, escalating roadblocks as required Lead change management activities and coach business leaders on their role in managing change on the project. Utilize Microsoft Office tools (Word, Excel, Teams) to maintain logs, manage actionable items, and support clear documentation and communication. Formally close the project, ensuring the completion of major deliverables and achievements of business goals. Conduct and document a lessons learned assessment.
