Clinical Operations Lead Primary & Community Care Program

Position Summary
The Clinical Operations Lead - Primary Care provides program-wide operational leadership across five Family Health Team (FHT) sites. Reporting to the Senior Clinical Program Director, Primary Care and Community, this role ensures consistent alignment of operations, quality improvement, and administrative processes across sites.
The Clinical Operations Lead is a key leadership role that bridges administrative and clinical priorities. The successful candidate will oversee clerical coordination, standardize workflows, support staff development, and advance quality improvement initiatives to deliver efficient, coordinated, and equity-focused primary care.
Key Responsibilities
- Provide operational leadership across five FHT sites, ensuring day-to-day consistency, workflow optimization, and alignment of clinical and administrative processes.
- Directly supervise Clerical Coordinators and provide coaching, mentorship, and performance support.
- Standardize onboarding and orientation processes for new staff across the program.
- Support planning, implementation, and evaluation of FHT-wide programs (e.g., chiropody, physiotherapy, nurse foot care), including collaborations with the Downtown East Toronto Ontario Health Team (DET OHT) and other external partners.
- Collaborate with Clinical Nurse Specialists, QIDSS, EMR Specialists, and program leads to monitor performance metrics and advance quality improvement initiatives.
- Support policy development, revision, and implementation across sites to ensure consistent practice and compliance.
- Provide project management leadership for new initiatives and support the alignment of operations between St. Michael’s FHT and the Urban FHT.
Qualifications

Regulated Health Professional with a Bachelor’s Degree in Nursing/Health Discipline required (Master’s degree preferred)

Registration in good standing with a regulatory college

Minimum 5 years of progressive leadership experience in healthcare operations, preferably in primary care, community health, or ambulatory care.

Demonstrated experience leading multi-site teams and operational functions.

Strong knowledge of primary care models, interdisciplinary team structures, and quality improvement methods.

Proven ability to manage complex projects, standardize processes, and implement system improvements.

Excellent communication, collaboration, and problem-solving skills.
