DUTIES AND RESPONSIBILITIES:

The Geriatric Nurse Practitioner is a key member of the Geriatric Consult team. Working with the inter-professional team and Physician partners the Nurse Practitioner (NP) will provide leadership in specialized comprehensive geriatric services across all programs and five sites. Partnering with Geriatrician and Behavioural Support Ontario Registered Nurses the Geriatric Nurse Practitioners will provide assessments and recommendations for Seniors admitted to Lakeridge Health. The Nurse Practitioner will leverage leadership, education, advocacy and research skills to improve patient outcomes, build capacity and foster system change. This position is ideal of a Nurse Practitioner (NP) with a passion for geriatric care.

The NP is responsible for:

  • Acting as a consult for Lakeridge Health Geriatric patients by promoting excellence in geriatric care through the roles of consultant, educator, researcher and leader;

  • Supporting the advancement of geriatric care through the provision of comprehensive geriatric assessments and, as appropriate, providing diagnosis, treatment and management plans to address the needs of older adults and their caregivers;

  • The NP consultation will involve health assessment and diagnosis of medical and cognitive problems, including the developing and implementing of plan of care in order to facilitate safe and efficient transitions;

  • Providing leadership and guidance for initiatives and planning related to geriatric care.

  • Working in collaboration with consulting Geriatricians to facilitate early assessment and intervention for patients requiring geriatric consults.

  • Identifying opportunities to enhance care practice across geriatric care and be an active participant with a geriatric lens in policy and procedure development.

QUALIFICATIONS:

  • Completion of a recognized Primary Care or Adult Nurse Practitioner program;

  • Masters of Nursing;

  • Current certification with the College of Nurses of Ontario - Extended Class

  • CAN Certification in Geriatrics would be an asset

  • BLS required

  • Additional specialty education in Gerontology preferred, such as: P.I.E.C.E.S.TM, Gentle Persuasive Approachj, Montessori Methods for Dementia

  • Experience and/or education in Palliative care is an asset

  • 1-3 years of demonstrated experience as a Nurse Practitioner required

  • 3-5 years experience in seniors care

  • Demonstrated in-depth expertise regarding clinical assessment and acute care needs of the vulnerable elderly

  • Demonstrated ability to work in a self-directed manner with well developed leadership, critical thinking, organizational and problem solving skills

  • Demonstrated strong communication and interpersonal skills with the ability to work collaboratively

  • Demonstrates extensive knowledge and skill working with individuals with various types of Major Neurocognitive Diseases, Behaviour and Psychological Symptoms of Dementia (BPSD), delirium and addictions

  • Valid Ontario Drivers License, a safe and operational vehicle and an ability to travel to all five Lakeridge Health sites

  • Must have satisfactory performance record;

  • Demonstrated understanding of and commitment to Lakeridge Health's comprehensive safety programs and practices including staff, patient and environmental safety and participation in regular in-services in this area

CONDITIONS OF EMPLOYMENT:

The terms and conditions of employment are in accordance with the Collective Agreement between the Hospital and the Ontario Nurses Association (RN Bargaining Unit Local 051).

Successful External applicants must provide a satisfactory Criminal Investigation Reference for the vulnerable sector, dated within the past six (6) months.
