About the role
The Dietitian provides nutritional care and develops, delivers, and evaluates nutrition-focused programs and services for individuals, families, and communities within a primary health care setting. The role emphasizes health promotion, food security, community development, and client education, especially among marginalized and diverse populations.
Program-Related Activities
- Conducts nutritional assessments and develops individualized nutrition care plans in line with College of Dietitians of Ontario standards.
- Assesses clients’ nutritional health considering cultural, socioeconomic, psychological, and clinical factors, as well as food literacy.
- Designs, implements, and evaluates nutrition-related treatment, education, and health promotion programs for individuals, families, and communities.
- Develops and delivers community-specific nutrition promotion programs based on research and needs assessments.
- Educates groups on nutrition, emphasizing health promotion and illness prevention through a self-management approach.
- Conducts community nutrition and food security assessments; supports initiatives to address food access barriers.
- Creates and maintains culturally and financially appropriate nutrition resources.
- Regularly evaluates programs and services, recommending improvements as needed.
