About the role

Summary of Duties:

Provision of Social Work services to clients and families in Odette’s Patient & Family Support Program, caring for patients with gastrointestinal cancer and haematology cancer at an advanced level of practice within an interprofessional model. This includes: completion of psychosocial assessments; leading or participating in Advance Care Planning and/or Goals of Care conversations; provision of counselling and emotional support; conducting group work; locating and arranging resources; consultation/collaboration; teaching; participating in research; administration and professional development.

Qualifications/Skills:

-Masters Degree in Social Work required. -Must be a current member in good standing with the OCSWSSW.

  • Minimum 5 years’ previous experience working in a health care setting preferred.
  • Demonstrated Psychosocial Oncology knowledge preferred.
  • Psychosocial provision of care to those with gastrointestinal, haematology cancer preferred.
  • Proven excellent assessment, counselling and problem solving skills essential. -Superior communication, negotiation, mediation and collaborative interprofessional team skills required. -Well-established knowledge of relevant health care system processes and community resources required.
  • Superior time management, organization, and prioritization skills in addressing multiple tasks and timelines in a fast-paced environment required.
  • Proven commitment to person and family-centred care essential.
  • Demonstrated strong commitment to academic practice required.
  • Student education required
