About the role

Overview Location: SW, Calgary.

Job Summary / Purpose The Licensed Practical Nurse (LPN) is responsible for delivering safe, competent, and person-centered nursing care to clients in their homes, in accordance with policies and procedures, the client's individualized care plan, physician’s orders, and the standards of practice set by the College of Licensed Practical Nurses of Alberta (CLPNA). The LPN collaborates closely with the care team and plays a critical role in supporting health, independence, and quality of life for clients.

Responsibilities Essential Duties and Responsibilities: Utilizing Sound Nursing Practice Principles, The LPN Will Ensure The Delivery Of Excellent Nursing And Personal Care In The Assigned Home Area(s)

  • Deliver safe and effective nursing care based on the assessed needs and care plan of each client.
  • Administer medications and treatments as prescribed, ensuring proper documentation and adherence to agency protocols.
  • Safeguard all medications and narcotics, ensuring compliance with standards of practice and agency policy.
  • Maintain accurate, timely, and complete nursing documentation in accordance with agency standards.
  • Monitor client health, observe and report changes in condition to the appropriate healthcare provider or supervisor in a timely manner.
  • Utilize proper techniques and equipment for client care
  • Follows all corporate policies and procedures; contributes to the development of site-specific nursing policies as indicated
  • Respond appropriately in urgent or emergency situations and escalate concerns as needed.
  • Adhere to all privacy and confidentiality regulations, including those under the Health Information Act (HIA)
  • Contribute to ongoing quality improvement and client satisfaction initiatives.
  • Flexibility to take on other assignments as required.
  • Additional duties and responsibilities as assigned by your Supervisor.

Qualifications Qualification: To perform this job successfully, an individual must be licensed and in good standing with the CLPNA, demonstrate the ability to carry out nursing responsibilities in alignment with legislation, professional standards, and agency policies, and successfully complete all required orientation and training, and demonstrate ongoing competency. The requirements listed below are representative of the knowledge, skill, and/or ability required.

Requirements

  • Active LPN license (CLPNA) in good standing.
  • Ability to communicate fluently in English (written and verbal)
  • Ability to adapt to and prioritize client’s needs
  • Ability to remain calm and professional under pressure
  • Clear Criminal Record Check with Vulnerable Sector Search (dated within 12 months).
  • Proof of Eligibility to work in Canada
  • Two professional references

Non-Required Preferred Skills, Education And Certifications

  • Previous experience in a leadership or supervisory nursing role.
  • Valid driver's license with a registered vehicle that is insured
  • Current CPR and First Aid Certification (recognized by Alberta Occupational Health & Safety).
  • Have a smartphone
  • Minimum of 1 year of nursing experience, preferably in home care, geriatrics, or community health.
  • Experience in palliative care and dementia care.
  • Experience in medication management, wound care, catheter care, G-tube, and colostomy management.

Physical Requirements

  • Must have a reliable means of transportation to get to and from work.
  • Ability to safely perform client transfers with weight bearing requirements contingent upon client condition and needs
  • Ability to sit, stand and walk for prolonged periods of time throughout the work day
