(rate effective October 1, 2025) Hours per Week: 35 hours per week Position Location: Clonsilla Paramedic Base (1003 Clonsilla Ave. Peterborough) Position Group: Unionized, CUPE 1306 Inside Competition #: 1306-26-11 Job Description: Patient Navigator What you'll do: The Patient Navigator supports the Community Paramedic Program by helping patients access appropriate health, social, and community services. Working collaboratively with community paramedics, primary care providers, hospitals, and community partners, the role enhances care coordination, reduces barriers, and supports improved health outcomes for unattached, complex, and vulnerable patients. Care Coordination & Patient Navigation: Coordinates referrals and supports patients in navigating the healthcare and social service systems. Conducts needs assessments, connects individuals to appropriate resources, facilitates care transitions, and helps patients access primary care. Collaborate & Develop: Builds strong partnerships with healthcare providers and community organizations to support coordinated, cross-sector care, and contributes to interdisciplinary case discussions and care planning. Administration: Maintains accurate records of patient interactions and outcomes, monitors progress for reporting purposes, and supports program evaluation, quality improvement, and administrative functions as required. In accordance with County Policy, a Police Record Check that is acceptable to the County will be required as a condition of employment.

This job is for you if… Our non-negotiables: You have a diploma or degree in a health related discipline (e.g. practical nursing, nursing, social service work), and a valid "G" class driver's license. Experience: You have 3 years of related job experience in a healthcare, community, or social services environment. Patient navigation, case management and care coordination would be considered an asset. Care-Advocate: you possess strong knowledge of the social determinants of health and community-based supports. People-Oriented: You possess exceptional decision making abilities and customer service skills with a focus on patient advocacy. Problem Solver: You excel at problem-solving and navigating complex systems and coordinating care across multiple providers.
