Accountabilities Job Description: Case Work/Facility

  • Maintain an active caseload.
  • Responsible for the day-to-day shift duties and reporting requirements.
  • Complete intake and assessments, creating service plans, providing case management services and clinical interventions for an assigned caseload.
  • Maintaining ongoing responsibility for primary client housing-stability planning and housing-related
  • case work.
  • Record activities and incidents occurring during the shift in a factual and concise manner using Logbook and/or Incident Occurrence Reports.
  • Contact emergency service personnel (ambulance or police) when necessary.
  • Maintain confidentiality with regard to clients/staff and Centre’s records.
  • Develop a network and make referrals to existing services and resources in the community including those of other Salvation Army departments.
  • Maintaining personal HIFIS account and using HIFIS system for all aspects client case work.
  • PSCW staff members are responsible for moderate cleaning and upkeep of facilities and resident quarters when property services staff are not on site.
  • PSCW staff are responsible to adhere to all tasks and responsibilities as outlined on the Front-Line worker responsibilities shift sheets.

Administrative

  • Responsible for reporting monthly PSCW performance statistics.
  • HIFIS -ensuring daily updates- submissions- assessments- incidents-housing plan- discharge.
  • Completing daily front-line worker responsibility logs and bed board charts.
  • Performing all relevant documentation related to PSCW including but not limited to: intake/discharge forms, client HIFIS notes, shift logs, incident reports, client forms, medication documents, client applications, stats reports, etc.
  • Responsible for maintaining personal MyArmy work account/password for all strategic work purposes including email, workday(attendance and scheduling), Salvation Army corporate information systems, Microsoft teams, etc. This is done with support from management, supervisory staff and I.T support personnel.

Perform Other Duties As Required. MANAGERIAL/TECHNICAL LEADERSHIP RESPONSIBILITY

  • Reports directly to: Program Services Coordinator
  • Indirect report to facility Program Director

Financial And Materials Management

  • Following procedures on donation processing and drop-safe depositing.
  • Balancing petty cash at each shift change client

Working Conditions

  • PSCW staff members work onsite at Sutton Youth Shelter
  • Minimal travel required
  • Shift schedules -12 Hour shifts
  • PSCW staff members work within the facility building, and outside of the building, onsite.

Education, Qualifications And Certifications

  • College diploma or equivalent post-secondary
  • Bachelor of Social Work is preferred.
  • Valid Driver’s License is a requirement.

Experience And Knowledge

  • Experience working with homelessness, mental health and addictions.
  • Must have functional knowledge and understanding of related to homelessness, mental health and addictions.

Skills And Capabilities

  • Thorough knowledge and understanding of and a commitment to anti-racism, anti-oppression and equity.
  • Strong oral and written communication skills are essential.
  • Ability to work independently as well as part of a team
  • Must have good interpersonal/time management skills
  • Good knowledge of community resources

Successful Candidate Will Be Required To Provide Upon Hiring

  • A clear vulnerable sector screening
  • Driver's abstract where required

This position is unionized - must be willing to work within a unionized environment

Compensation The target hiring range for this position is $20.42 to $20.42 with a maximum of $20.42.

Placement in the salary range will be based on factors such as market conditions, internal equity, and candidate experience, skills, and qualifications relevant to the role.
