SUMMARY:

The Retail Pharmacist is responsible for the safe, effective, and patient centred delivery of pharmacy services in accordance with the Drug and Pharmacies Regulation Act, Ontario College of Pharmacists (OCP) standards, and organizational policies. The incumbent provides clinical pharmacy services, medication counselling, dispensing, and professional oversight, and collaborates with inpatient hospital pharmacists, physicians, and interprofessional teams to support medication and medical device needs across transitions of care, including hospital discharge and follow-up.

RESPONSIBILITIES:

  • Assess prescriptions for appropriateness, safety, and therapeutic effectiveness; identify and resolve drug-related problems in collaboration with prescribers.
  • Provide patient centered medication counselling and education, medication reviews, minor ailment services, and injections, and authorized expanded scope services in accordance with Ontario legislation, QCH medical directives, training, and certification.
  • Support Work collaboratively with other pharmacists and physicians to support continuity of care, medication reconciliation, prescription clarification, and timely access to medication patient education to ensure continuity of care.
  • Work collaboratively with other pharmacists and physicians to support hospital discharge transitions through medication reconciliation, prescription clarification, and access to medication.
  • Support patients by reinforcing medication plans, addressing barriers to access, and ensure understanding of therapy changes initiated in hospital.
  • Accurately dispense and perform final verification of prescriptions and compounded products in compliance with legal, clinical, and professional requirements.
  • Ensure accurate documentation, handling, storage, and distribution of medications, including controlled substances and narcotics.
  • Support the dispensing, education, and coordination of medical devices and supplies, and ensure continuity and appropriateness of medical device therapy following discharge.
  • Respond to inquiries from hospital teams related to outpatient medication access, insurance coverage, and availability of medications or medical devices.
  • Provide guidance and support to pharmacy assistants, technicians, and students within scope and supervision requirements.
  • Practice in accordance with standards set by the Ontario College of Pharmacists, applicable legislation, and organizational policies.
  • Participate in quality assurance activities, medication incident reporting, and continuous quality improvement initiatives.
  • Maintain patient confidentiality and uphold professional standards.
  • Responsible for clinical aspects of inventory management, including drug selection and stock level planning based on patient care needs, prescribing trends, and therapeutic appropriateness.

REQUIREMENTS:

  • Graduate of a recognized Faculty of Pharmacy and hold a Bachelor's Degree of Science in Pharmacy
  • Licensed or eligible for licensure with the Ontario College of Pharmacists (Part A of Register)
  • Three (3) years relevant experience in outpatient/community pharmacy environment
  • Observe strict confidentiality of all patient and hospital-related information
  • Demonstrated effective communication, interpersonal, and organizational skills
  • Commitment to and responsible for continuing education and professional development related to position

PREFERRED:

  • Three (3) or more years of hospital experience
  • Experience with an Inpatient Electronic Health Record system (EHR)
