Posting Period: October 03, 2025 - October 09, 2025 Position Summary This position with the Providence Care – Transitions Home Program will be engaged in community work and involves commuting across the Southeastern Ontario region. The Transitions Home Program works at achieving timely access to care and clients’ functional outcomes following discharge from PCH/PTCC. Therapeutic interventions will be carried out in the client’s home and community.

The Occupational Therapist, reporting to the Clinical Program Coordinator, acts as the fundamental resource to program staff, clients, caregivers and community partners. In collaboration with the program staff, the Occupational Therapist provides assessment of assigned clients and develops and implements individual occupational therapy treatment programs. The Occupational Therapist is accountable for maintaining the Standards of Practice and Ethics of the College of Occupational Therapists of Ontario. The Occupational Therapist will provide clinical education and experience for allied health students and will participate in formal teaching and research in conjunction with Queen’s University. As a member of the clinical team, the Occupational Therapist participates in discipline-specific and inter-professional activities that are directed towards a client-centered environment and that contribute to quality, operational efficiency, professional and program development. As Occupational Therapist, the goal is to help clients be healthier at home while benefitting from our supports of rehabilitative care.

Required Qualifications And Experience

  • Graduate of a recognized school of Occupational Therapy
  • Active, unrestricted license to practice with the College of Occupational Therapists of Ontario
  • Experience in specified area of practice preferred
  • Experience in student supervision an asset
  • Minimum of two (2) years of relevant professional experience, preferred
  • Experience working with home care teams and coordinating community services.
  • Sound knowledge of rehabilitation best practices.

  • Utilizes evidence-based practice and best practice
  • Utilizes outcome measures to guide clinical decision-making
  • Identifies and implements treatment to achieve client goals
  • Assigns appropriate client care to support staff; monitors and supervises client treatment and progress
  • Assesses level of function and the needs of clients in the areas of self-care, productivity and leisure
  • Establishes treatment goals in collaboration with the client
  • Valid Ontario driver's licence and use of a reliable motor vehicle.
