TRAINING OPPORTUNITY

Candidates accepting this training opportunity agree to remain with Vancouver Coastal Health (VCH) in the area of nursing applicable to the education program for which they are hired for at least 18 months subsequent to completing training.
This will ensure that candidates are able to consolidate and apply skills in the specialty area that VCH has supported and funded candidates to complete. In the event that a candidate leaves the area of nursing applicable to the education program for which they were hired for VCH in under 18 months, VCH will seek to recover the cost of the program.

As a Community Health Nurse you will:
- Care for clients with acute, chronic and palliative care needs.
- Work in partnership with the client and family to establish goals that are safe, realistic, and reasonable by assessing, planning, coordinating, organizing, implementing, evaluating, and transitioning nursing and other services for the client.
- Assess service eligibility, facilitate the development, implementation, and adaptation of a care plan, deliver direct care, and provide care management to assist clients to manage their own care and navigate through the various services available within the community settings.
- Work in consultation with other system partners and incorporates current evidence into practice.
ABOUT HOME HEALTH PROGRAMS: Home Health programs provide a range of health care and support services to VCH to people living in the community who have acute, chronic, palliative or rehabilitative health care needs.
THE CLIENT POPULATION: Home Health clients typically have on‑going health issues and/or complex personal and/or social issues, requiring care planning and coordination of appropriate resources. Clients include frail elderly, younger clients with disabilities, and adults with chronic illnesses requiring supports to maintain/promote independence. Pender serves the Gastown, Chinatown, Strathcona, and Downtown Eastside neighbourhoods.
Qualifications
Education & Experience
- Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- One year recent related clinical experience in a relevant clinical area working with complex clients/patients with chronic health conditions and/or functional mobility issues including recent experience in care and transition planning, or an equivalent combination of education, training and experience.
- Valid BC Driver’s License required as local area travel may require the use of a vehicle or other accepted mode of transportation.
Knowledge & Abilities
- Demonstrated knowledge and skills in community health nursing theory and practice within a client and family centred model of care.
- Assessment and Intervention – Demonstrated ability to provide trauma‑informed, harm‑reduction, and culturally safe care.
- Critical Thinking – Ability to assess safety and risk prior to entering home environments and to integrate and evaluate data from multiple sources.
- Problem Solving – Ability to manage psychosocial and behavioral issues and employ effective conflict‑resolution approaches.
- Independence – Ability to manage and prioritize clients, promote cooperation, and pursue professional development.
- Resiliency – Ability to adjust to new or unexpected events and promote client‑focused care.
- Collaboration – Ability to work collaboratively as a member of an interdisciplinary team.
- Documentation – Ability to document timely and appropriate information in an electronic medical record (EMR).
- Confidentiality – Ability to maintain client privacy and confidentiality.
- Communication – Ability to develop rapport, trust and ethical relationships with clients, families and health professionals.
- Facilitation & Coaching – Ability to teach, facilitate and coach using adult‑education principles.
- Knowledge of provincial acts, regulations, and program policies related to home and community care.
- Knowledge of acute, chronic disease, palliative, mental health and substance use management.
- Knowledge of community resources available for client/family support.
- Knowledge of research, quality improvement, evaluation processes and self‑management support.
- Demonstrated ability to plan, organize, schedule and prioritize work.
- Demonstrated skill in CPR techniques and use of nursing equipment.
- Broad knowledge of BCCNM standards of practice.
- Ability to operate related equipment and computer applications.
- Demonstrated physical ability to perform the duties of the position.

The hours of work including days off and work area may be subject to change consistent with operational requirements and the provision of the Collective Agreement and applicable statutes.
As per Ministry of Health policy, all health care workers working in publicly‑funded health care facilities are required to report their past receipt of certain vaccines or history of certain infections. Collecting these records will allow for offering of any missing vaccines, and for appropriate actions to be taken in the event of any future exposure to a communicable disease or during outbreaks. For all new hires and appointments to Vancouver Coastal Health, you will be asked to provide this information as part of the onboarding process.
