About the role
JOB SUMMARY Reporting to the Manager of Education and Community Programs, the Health Promotion Facilitator is responsible for the development, coordination, and delivery of health promotion programs, education, and initiatives that support active living and brain health. This role ensures persons living with dementia (PLWD) and their care partners are engaged in recreation-based activities designed to maintain physical, social, and emotional well-being. The Facilitator works collaboratively within the client’s circle of care and with community partners to provide accessible programs in the community and in clients’ homes.
KEY DUTIES AND RESPONSIBILITIES
Program Development and Delivery Promote awareness and education on Alzheimer’s disease, related dementias, active living, brain health, and programs offered by the Society. Coordinate and deliver core health promotion programs such as Minds in Motion®, Urban Poling, Art Therapy, Friendship Circle, and others as assigned. Develop programs tailored to client interests and abilities, adapting activities or providing alternatives as required. Encourage and facilitate social networking and relationship building among participants. Evaluate programs to ensure quality standards and desired outcomes are achieved. Collect attendance data and other relevant program information to support evaluation and reporting to funders.Job ID: HPF-TEMP-2026, to awarrell@alzheimernorthbay.com, Attention: Ashlee Warrell Community Engagement and Partnerships Develop and maintain partnerships with recreation centres, older adult centres, and other community venues for program delivery. Act as a resource for the public regarding active living, brain health, recreation, and activation. Market health promotion programs to healthcare providers, referring agencies, and the general public. Contribute to internal and external committees and participate in fundraising or promotional events as required.
Volunteer and Staff Support Assist with the screening, training, and supervision of volunteers and post-secondary student placements supporting programs. Work collaboratively with Society staff, volunteers, and community partners. Ensure documentation of services is completed in a timely manner using assigned tools.
Other Duties Perform other duties consistent with the job classification as assigned by the Manager of Education and Community Programs or designate.
QUALIFICATIONS
Education Post-secondary degree or diploma in Recreation Therapy, Social Work, Gerontology, Health Studies, Dementia Studies, or related discipline. Other health disciplines may be considered with appropriate education and experience. Certification in good standing as a Fitness Instructor/Personal Trainer. Seniors Fitness Certification considered an asset.
Experience, Skills, and Knowledge 3 to 5 years of experience in health and/or social service sectors. Experience working with persons living with dementia and their families. Experience designing recreational programs to support individual needs and facilitating group activities. Knowledge of Alzheimer’s disease and related dementias. Excellent verbal and written communication skills. Bilingual (English & French) considered an asset. Strong presentation, public speaking, and interpersonal skills. Team-oriented with ability to work independently and meet deadlines efficiently. Initiative and resourcefulness in completing tasks. Proficiency in Microsoft Office Suite, PowerPoint, and statistical databases. Experience and expertise in cultural sensitivity and diversity. Current Standard First Aid/CPR certification. Valid Ontario driver’s license, regular access to a vehicle, and verification of insurance for work purposes.
SALARY RANGE Salary Range: $44,000 to $50,000 per year, commensurate with experience.

PHYSICAL DEMANDS General office duties. Facilitation of exercise and fitness programs. Extended periods sitting at a computer with opportunities to move away from the workstation. Periods of standing to deliver presentations and programming. Sustained sensory attention required for reading documents and screen work.
TRAVEL REQUIREMENTS Travel across the chapter district as required. Valid Ontario Driver’s License required.
