DEPARTMENT: DISTRICT STROKE POSITION LOCATION: OWEN SOUND UNION AFFILIATION: ONA STATUS: FULL-TIME REASON TO POST: DIRECT REPLACEMENT WAGE RATE: $40.24 – $57.68
Reporting to the Manager of Rehabilitation and Stroke Services, the Stroke Prevention Clinic Nurse (RN) provides expert, evidence-based nursing care focused on secondary stroke prevention, vascular risk factor management, and longitudinal follow-up for persons with stroke, transient ischemic attack (TIA), and those at high risk for recurrent stroke.
This role integrates advanced clinical nursing practice with community system navigation, ensuring coordinated, seamless care across the continuum from hospital-based services to community reintegration. As a member of the District Stroke Team, the Stroke Prevention Clinic Nurse acts as a clinical leader, educator, consultant, advocate, and resource for patients, families, care partners, health care providers, and community agencies.
The Stroke Prevention Clinic Nurse supports improved patient outcomes through education, coaching, care coordination, navigation, and quality improvement aligned with Canadian Stroke Best Practice Recommendations.
Skills/Abilities/Qualifications
Required
- Registered Nurse in good standing with the College of Nurses of Ontario
- Recent clinical experience in stroke care, neurology, cardiovascular care, ambulatory care, or chronic disease management
- Demonstrated knowledge of stroke prevention and secondary prevention strategies
- Strong assessment, care coordination, navigation, and case management skills
- Excellent written and verbal communication skills
- Ability to work independently and collaboratively across health care and community systems
- Recent satisfactory performance and attendance records
- Adherence to Brightshores ‘CARE’ Behaviours (Collaboration, Accountability, Respect, Excellence) and ‘LEAD’ Competencies (Lead by example, Empower, Achieve results, Develop others) and a commitment to a safe workplace free of violence and harassment
- Advocates and supports a culture of patient safety and demonstrates an understanding of how the department service and assigned responsibilities contribute to overall patient safety at Brightshores through patient safety knowledge, skills and attitudes
Preferred
- Baccalaureate degree in Nursing
- Canadian Neuroscience Nursing Certification (CANN) or willingness to obtain
- Experience in stroke prevention clinics, outpatient neurology, or community-based roles
- Experience in system navigation, care coordination, or integrated care models
- Quality improvement or change management experience
