POSITION SUMMARY

The Case Manager, Social Worker (Family Specialist) works collaboratively with the members of the interdisciplinary team to provide education, assessment, support, and advocacy primarily for the client, family members and friends/significant others of clients experiencing a first episode psychosis. The Family Specialist will screen referrals, conduct assessments and participate in the development of treatment plans that not only mitigate the effects of the first psychotic episode for the client, but also address the impact of the illness on family. The Family Specialist will implement clinical best practices, engaging in therapeutic partnerships with clients, their families, the hospital and community resources.

RESPONSIBILITIES:

  • Conduct and/or actively participate in screened client interviews.
  • Conduct interviews with family members to obtain pertinent history relevant to assessment and treatment of ill relative.
  • Develop, implement and evaluate treatment agreements.
  • Use recovery and rehabilitation models in assessment, planning, intervention and evaluation of family centred interventions.
  • Espouse and enact the principles of therapeutic partnership.
  • Provide psychoeducation to clients, families and community partners with a focus on helping them to understand episodes of psychosis (e.g. etiology, treatment, medication, prognosis, recovery).
  • Liaise and advocate between the client, family, other members of the treatment team and the larger treatment community.
  • Provide crisis intervention and support.
  • Provide interventions which reflect the unique needs of young adult clients and their families.
  • Reinforce the strengths and adaptive coping strategies of family and friends.
  • Participate in group planning, implementation and evaluation as needed.
  • Demonstrate a commitment to professional and team oriented education.
  • Provide education to health science students and professionals
  • Offer public/community education.
  • Participate in program evaluation, best practice and research.
  • Develop and facilitate group programs for clients and family members as needed.
  • Provide regular written and phone communication with client’s family.
  • Attend supervision, team, administrative and relevant system-level meetings.
  • Complete required documentation and collect statistical data in a timely manner.
  • Demonstrate ability to build and maintain statistical programs related to outcome measures
  • Consider safety of self and co-workers while performing work.

QUALIFICATIONS:

  • M.S.W. or equivalent with significant experience in the field of mental health and addictions and family work preferred.
  • Knowledge and minimum 3-5 years experience working in an outpatient/community setting with persons with psychotic disorders including etiology, prognosis and treatment modalities.
  • Skills, knowledge, and attitudes commensurate with the provision of intensive clinical case management services and crisis intervention functions.
  • Ability to assess efficacy and side effects of many different psychiatric medications: mood stabilizers, antipsychotic medications, and antidepressants.
  • Knowledge of the unique developmental issues of young adult clients and their families.
  • Knowledge and experience working with clients with substance use issues.
  • Knowledge and experience working within a psycho educational model with families.
  • Knowledge of family systems theory, the adaptation process in families, adjustment to chronic illnesses, utilizing the principles of adult learning.
  • Experience in health teaching strategies for clients, families and community.
  • Ability to work in a collaborative partnership with clients, family and community.
  • Ability to model advocacy skills and coach clients and families to acquire these skills.
  • Communication skills, interviewing skills, critical thinking, and problem solving skills.
  • Knowledge, skills and experience in the principles and practices of psychosocial rehabilitation, psycho education and recovery including hope and empowerment.
  • Participation in research, best practice, program evaluation, and evidence of continued professional development and education.
  • Knowledge of community resources to support clients and families.
  • Computer skills in Microsoft Outlook, Windows, Word, Excel and PowerPoint.
  • Flexibility and responsiveness to shifting service demands.
