Role Summary

Reporting to the Director of Care or designate, and working as part of a multi-disciplinary team, the Care Services Lead will work closely with front line team members to provide high quality care and services to residents. Under the general supervision of the Director of Care, this Care Services Lead is responsible for assessing, coordinating, supervising, administering, and evaluating resident care and services.

Responsibilities

  • To collaborate with the site leadership team, residents, families, and other stakeholders in meeting the short and long term needs of all residents.
  • To lead the development, implementation, evaluation, and revisions of care related goals, objectives, and outcomes.
  • To provide professional clinical leadership to the care team.
  • To provide oversight and direction to the care team, equipping them to meet the day-to-day care needs of all residents under the resident centered model of care.
  • To provide direction and leadership to the clinical team, including communication and interpretation of policies and procedures. Ensure the proper maintenance of all resident care records and other records in accordance with policy and relevant legislation.
  • As part of the management team will participate in the budgeting process.
  • As a member of the OH&S Committee, ensure the care home, resident and team members activities are maintained and conducted in accordance with safety standards.
  • Together with the General Manager and appropriate leadership team members, ensures all team members are properly trained and orientated.
  • Identifies areas of educational needs; develops, organizes and conducts in-service sessions including those provided by outside professionals.
  • Ensures all incident reports are properly reported, documented and that all appropriate action is taken in this regard.
  • Ensures that all care team members maintain personal good health habits and meet the physical and cognizant requirements of their respective jobs.
  • Other duties as assigned.

Qualifications and Experience

  • Successful completion of a Practical Nursing degree together with a current registration with a provincial professional body.
  • Minimum five (5) years recent nursing experience, preferable in Residential Care and or Home and Community Care, involving supervision of a multidisciplinary team members budgetary responsibilities and continuous quality improvement processes.
  • Knowledge and ability to plan, direct, evaluate and control multi-faceted care within a resident centered model of care and service delivery.
  • Have effective multi-tasking, time management and work prioritization abilities.
  • Must demonstrate the ability to communicate effectively and maintain cooperation with all levels of personnel.
  • Must be able to deal with residents and their relatives in a manner that demonstrates tact, empathy and patience.
  • Must possess the qualities and skills to act as a role model and leader to Nurses, Care Aides and other Hamlets team members.
  • Computer literacy in Windows, including Word, Excel and PowerPoint.
  • Must have the temperament necessary to maintain a caring manner while demonstrating a therapeutic approach to the delivery of care.
