Position Summary

Reporting to the Program Manager, Safe Centre of Peel, the Client Navigator is responsible for acting as the point of first contact for program participants, potential clients and community. The Client Navigator conducts intake, and screening; provides crisis intervention, short-term counseling intervention and advocacy to individuals, families, and make appropriate referrals to community services. The incumbent also establishes and maintains relationships with on-site and off-site partners, agency service partners and referral sources. The incumbent is also responsible for conducting telephone and in-person outreach activities and completing all appropriate administrative records in a timely manner.

Responsibilities

Triage, Assessment and Case Management

  • Answer client inquiries regarding services and support client in providing therapeutic triage and assessment.
  • Ensure all attempts of client contacts are documented.
  • Conduct initial triage/assessment through various avenues such as face-to-face, telephone and email to determine client needs, interests, eligibility criteria and next steps.
  • Conduct therapeutic, trauma-informed risk assessment and safety planning
  • Facilitate referrals to on-site and off-site partners, agency programs, other professionals, agencies, services, or community resources appropriate to clients identified needs.
  • Respond to telephone, email, and in-person inquiries from clients, prospective clients and community partners.
  • Process and maintain internal and external referrals.
  • Assist in the co-facilitation of group programs.
  • Follow up and provide ongoing support with high-risk clients
  • Liaise with on-site and off-site partner agencies that have made referrals to the agency
  • Provide telephone support, crisis intervention, safety planning and follow-up for clients
  • Provide case management as assigned.

Community Involvement

  • Positively and professionally represent the agency in the community by collaborating with community partners on various projects and initiatives as assigned.
  • Represent the agency at internal and external committees as needed

Administration

  • Prepare, compile, submit, and maintain accurate records, files, statistics, and additional relevant information in accordance with agency policy, and funding requirements.
  • Keep careful and accurate records and maintain client case file of all relevant information pertaining to assigned caseload.
  • Collect relevant client demographic data.
  • Create, update, and maintain client enrollment information in database in a timely manner
  • Train and support new staff, students, and volunteers.
  • Maintain and update agency on-line telephone logs.
  • Maintain data collection spreadsheets.

Other

  • Participate in agency fundraising activities as deemed appropriate and necessary by Senior Management.
  • Attend required agency activities and meetings.
  • Perform other duties as assigned.

Qualifications

  • Bachelor's Degree in Social Work, Community College Diploma or related human service field and two (2) years of relevant work experience preferred.
  • Knowledge of the principles and practices of counseling, social work, and case management.
  • Demonstrate positive interpersonal skills and ability to work in a team environment with co-workers, community providers and stakeholders, and clients.
  • Strong organizational and time management skills, ability to prioritize and meet multiple deadlines.
  • Excellent verbal and written communication skills in English (French as well as other languages an asset).
  • Proficient in Microsoft Office suite and working knowledge of database management.
  • Ability to work independently.
  • A satisfactory Vulnerable Sector Criminal Records check.
  • Valid driver’s license, access to a reliable vehicle and relevant insurance.

Working conditions

  • Work is performed primarily in a standard office environment.
  • Some travel will be required to satellite offices for agency meetings.
  • Evening work is required.

Physical requirements

  • The incumbent will have to spend long hours sitting and using office equipment and computers. The incumbent may also have to do some light lifting of supplies and materials from time to time.
  • The incumbent in this position frequently communicates with internal and external stakeholders who require information or need support. Must be able to exchange accurate information in these situations.
