Program Overview – Mental Health & Addictions, Community Care
The Social Work/Full Circle Psychogeriatric teams provide community-based case management, supportive and/or clinical counselling to older persons (55+) with mental health and/or substance use issues, cognitive issues (i.e. dementia), frailty, and other psychogeriatric conditions related to the aging process.
The Supervisor role will include bringing expertise in concurrent disorders and harm reduction to the team. This position will be responsible for providing clinical supervision to registered professionals, staff supervision, consultation, training, supporting annual program planning and evaluation, compliance with relevant legislation and organizational policies and representing the Social Work and Full Circle programs within the community, the organization, and external committees and partnership. This position is further responsible for monitoring, evaluating, and ensuring that the programs and funder outcomes are met.
What You Will Do

Provide clinical supervision and consultation support to Social Work and Full Circle staff.

Support program manager with recruitment, interviewing and retention of staff.

Develop and/or update program manuals and materials to ensure compliance with organizational, funder-specific, and accreditation standards and requirements.

Update and maintain Social Work and Full Circle Sharepoint folders with relevant resources and tools for the team
-
Oversee compliance with documentation standards and provide training to staff on client documentation
-
Provide/organize orientation and training for new Social Work and Full Circle staff.

Provide/organize supervision for practicum students in the Social Work and Full Circle programs.
-
Maintain regular contact with relief/casual staff, providing supervision support and clinical consultation.

Be familiar with the policies and procedures and the site-specific emergency procedures
-
Provide timely crisis intervention support to staff and/or practicum students
-
Monitor staff performance targets and data quality by regularly reviewing documentation in client management systems and consulting with staff and program manager
-
Support manager with preparing statistical reports, program reports and submissions.

In collaboration with program manager, supervise and manage any performance issues.

Maintain healthy team dynamics through team building activities, overseeing coverage arrangements for vacations/leaves, caseload review and other supportive interventions

Coordinate Albany Clinic drop-in and other community drop- ins as assigned

Participate in ongoing training and education to maintain and enhance skills in this role

Participate in regular supervisory sessions and annual performance appraisals with the program manager

Oversee waitlist management ensuring seamless assignment of client referrals
-
Lead team meetings when needed.

Participate in unit management meetings for the purpose of ensuring coordinated and integrative programming and resource sharing

Represent the unit in internal/external committees/conferences/collaborations as directed by program manager.

In collaboration with program manager, ensure that the equity objectives and community development policies of the organization are achieved within the programs.
What You Bring to the Team
-
Masters of Social Work (M.S.W.) or graduate degree in a relevant field.

Registered Social Worker (R.S.W.) in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) and/or Member in good standing with a Professional College in Ontario.

5+ years related experience in mental health, addictions, geriatrics and/or social work. Demonstrated experience working with older adults and seniors with mental health (including dementia) and/or substance use issues, chronic health issues, cognitive impairment, and responsive behaviours is required.

Minimum 3 years progressive leadership experience preferably in the Community sector and unionized environment.

- Knowledge of issues affecting older adults/geriatric populations, mental health, addictions, health sector, and community resources, policies, legislation, programs, and other issues related to scope of practice and social determinant of health.
- Solid understanding of the intersectionality of systemic barriers and social determinants of health, including racism, discrimination, and oppression.
- Demonstrated experience in group facilitation (therapeutic and psycho-social).
- Strong assessment, clinical and intervention skills.
- Effective oral, written communication skills and documentation skills.
- Effective interpersonal, crisis management, time management and organizational skills.
- Proven ability to apply and practice Anti-Oppression, Anti-Racism, Trauma-Informed and Harm Reduction principles, and strategies.
- Applied computer skills (MS word, excel, sharepoint, google docs) and ability to use client information systems.
- Ability to communicate in a second language (Greek, Spanish, Bengali, Urdu, Tamil, Farsi, Tagalog, Arabic, Mandarin, or Cantonese) is a strong asset.
