Speech-Language Pathologist, Home and Community Care - Temporary Full Time

POSITION SUMMARY
The Speech-Language Pathologist (S-LP) will work as a member of the School-Based Rehabilitation Services (SBRS) team, providing speech language therapy services to CHEO patients aged 4 - 21 years of age in a variety of settings including homes, schools and other areas as needed. The S-LP will conduct clinical assessments and develop treatment plans and goals in consultation with the patient, family and other interdisciplinary team members, in accordance with the standards of the College of Audiologists and Speech-Language Pathologists of Ontario (CASLPO). The Speech-Language Pathologist provides recommendations for follow-up and management of the patient’s condition.
This position will provide a range of client-focused interventions for children and their families including but not limited to education, assessment, individual and group treatment in alignment with the clinical pathways current in place for speech therapy. This role may also support the creation of additional therapeutic pathways. The School-Based Rehabilitation Service supports speech needs including fluency, articulation, motor speech, phonology, voice and resonance.
These services may be provided virtually or at one of CHEO’s locations across Eastern Ontario, or in schools across the Champlain region. An appropriate work from home environment, ensuring the safety and privacy of health records and information is required, as is a stable internet connection capable of supporting smooth, interruption free clinical appointments with families and patients. Due to the hybrid nature, travel will be required to various CHEO sites, community locations and/or schools. Working hours could vary from 8:00am to 4:00pm, or 10:00am to 6:00pm.
LOCATION (S): Travel throughout all of Ottawa may be required. Including the ability to travel to provide service to patients throughout Ottawa, Ottawa West (Kanata), Ottawa East (Orleans/Cumberland), Nepean, Barrhaven and all points in between.
MAIN RESPONSIBILITIES
- Completes assessments of patients in a variety of treatment settings including private homes and schools, and performs an analysis based on objective, subjective and functional findings.
- Establishes realistic, measurable and time limited treatment goals and develops treatment plan.
- Ensures that speech-language therapy treatments are initiated with the informed consent of the patient or substitute decision maker in accordance with the Health Care Consent Act.
- Provides direct treatment and/or support and teaching to informal caregivers, school personnel and other health care professionals involved.
- Participates in case conferences when required and consults with other members of the health care team in fostering a holistic approach to care.
- Completes reassessments to review patient's progress and treatment goals and reports to Care Coordinator and other members of the health care team in order to ensure continuity of care and determine if other resources and referrals are required.
- Plans in collaboration with the patient, family and Care Coordinator, for the termination of service when patient's goals are achieved and/or when patient no longer requires the service.
- Completes documentation and maintains appropriate patient records in accordance with the standards of College of Audiologists and Speech-Language Pathologists of Ontario (CASLPO) and CHEO policies and procedures.
- Responsible for teaching patient, family and other team members, the treatment program and management.
- Acts as a resource for CHEO employees, service providers, school personnel, and community by providing educational programs as requested.
- Attends and participates actively in team meetings and general staff meetings.
- Assumes responsibility for maintaining professional knowledge and updated skills.
- Prioritizes Speech Language Therapy services in accordance with the CHEO policy.
- Perform work in accordance with the provisions of the Occupational Health and Safety Act and Regulations and all CHEO corporate/departmental policies and procedures related to Occupational Health and Safety.
- Perform other related duties as assigned by supervisor.
QUALIFICATIONS
- Master’s Degree in Speech-Language Pathology (Essential)
- Registration with the College of Audiologists and Speech-Language Pathologists of Ontario (CASLPO) (Essential)
- Minimum two (2) years’ experience as a Speech-Language Pathologist, preferably in pediatrics (Essential)
- Related experience in a community-based setting or other SBRS Program (Preferred)
- Experience conducting patient assessments virtually (Preferred)
- Current First Aid/CPR Certification (Preferred)
- Excellent time management and organizational skills (Essential)
- Comfort level and skill in preparing and facilitating therapeutic groups (Essential)
- Highly level of accuracy and attention to detail, particularly with medical documentation (Essential)
- Excellent communication and interpersonal skills, especially while communicating with families, patients, educators and other partners (Essential)
- Able to work independently and as part of a team. This role is highly independent (Essential)
- Valid Ontario’s driver’s license, access to a vehicle, and the ability to travel from site to site and to multiple community-based visits on a daily basis is required. (Essential)
- Ability to travel to provide service to clients throughout all of Ottawa, Ottawa West (Kanata), Ottawa East (Orleans/Cumberland) (Essential)
- Ability to attend last-minute meetings in-person and/or virtually (Essential)
- Flexible hours including evening, weekend, and holiday work may be required.
