KEY FUNCTION**:**

Provide safe and effective therapeutic exercise treatment and education for patients with chronic pain in efforts to maintain functional capacity and level of independence.

REPORTING**:**

Under the general direction of the Clinical Manager, Cardiac and Pulmonary Rehabilitation, Integrated Chronic Pain, Chronic Disease Self-Management, HAVEN/Hemophilia/Hemoglobinopathy, Outpatient Brain Injury Rehabilitation Program and Outpatient Neurological Rehabilitation Program.

DUTIES**:**

  • Oversee therapeutic exercise sessions for outpatients of the Integrated Chronic Pain Program and offer rehabilitation counselling.

  • Design and oversee exercise sessions, develop exercise prescriptions based on the results of a graded exercise test, and participate in emergency responses.

  • Provide initial and follow-up assessments, goal setting, and motivational support to clients and families regarding long-term behavioural modification.

  • Document within an electronic Patient Care System (PCS).

  • Act as a Kinesiology preceptor for post-secondary students, and design and deliver group education sessions in a classroom setting and via videoconference.

  • Participate in departmental research projects and maintain workload measurement statistics.

  • Refer patients to other members of the interdisciplinary team, and participate in case conferences, team huddles, and team meetings.

  • Liaise and work with other agencies and physicians’ offices to coordinate patient care as required.

  • Evaluate patient care and satisfaction on an ongoing basis and formulate solutions for improvement.

  • Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.

  • Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.

  • Educate and promote health, safety and wellness in the work place.

  • Represent the department or program on various committees and in meetings as required.

  • Perform other duties as required.

QUALIFICATIONS

EDUCATION AND TRAINING:

  • Minimum of a four (4) year Bachelor’s Degree in Kinesiology or Human Kinetics, from an accredited university.

  • Current Certificate of Registration within the Ontario Kinesiology Association (OKA).

  • Current Certificate as a Clinical Exercise Specialist (CEP) through the American College of Sports Medicine (ACSM) or willing to obtain within two (2) years.

  • Current Certificate in Basic Cardiac Life Support (BCLS) and Automated External Defibrillator (AED).

  • Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.

EXPERIENCE:

  • Minimum of one (1) year full-time equivalent experience working in exercise interventions with patients within the last five (5) years.

  • Minimum of one (1) year full-time equivalent experience working in adult education within the last five (5) years.

  • Minimum of one (1) year full-time equivalent experience in providing rehabilitation care and education to patients living with chronic pain.

KNOWLEDGE/SKILLS/ABILITIES:

  • Demonstrated ability to actively participate in exercise leadership, including hydrotherapy in a pool environment and resistance training.

  • Demonstrated ability to work in an integrated team.

  • Demonstrated planning, analytical, and problem-solving skills.

  • Demonstrated experience using virtual platforms (e.g. Ontario Telemedicine Network) to schedule and deliver patient care.

  • Demonstrated training, experience or utilization of lean methodology for process improvement.

  • Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.

  • Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems.

  • Demonstrated superior interpersonal and communication skills, both written and verbal.

  • Demonstrated ability to manage routine correspondence, multiple tasks/projects, diversified workload and rapidly changing priorities and challenging deadlines.

  • Demonstrated discretion and maturity when handling confidential information.

  • Demonstrated commitment to the safety of co-workers and patients.
