Family Physician - Low-Barrier Homeless Outreach Team (300-1020)

The Homeless Outreach Support Team (IHOST) is looking for a Family Physician with Addictions Medicine interest to join its team.

The IHOST team is located in the Comox Valley Nursing Centre and provides team-based care in partnership with Mental Health and Substance Use teams. This role supports episodic care on an outreach basis at identified sites in the community providing primary care for those not yet able to successfully attend clinic-based services, to increase access. We provide primary care, wound care, addiction care and support to systemically marginalized populations. This position is well supported with a collegial physician team happy to trouble shoot. The team includes peer supports, clinicians and admin staff. This allows service five days weekly and same-day access. The Comox Valley Nursing Centre is a service within Island Health’s Primary Care portfolio - 'we believe in partnerships between clients, family and health care professionals.' Work is also available with the Addiction Medicine Consult Service at the Comox Valley Hospital.

  • Bi-weekly Thursday and weekly Friday coverage needed
