About the role

Position Summary:

Journey Home Hospice, a program of the Saint Elizabeth Foundation, provides exceptional end-of-life care in a residential hospice setting. Our Social Workers bring person-centered care, build rapport and trust with patients, and enhance their quality of life. You will join a collaborative and supportive care team, applying your skills and heart to meet the physical, cognitive, social, emotional, and spiritual needs of patients and families.

Responsibilities:

  • Conduct comprehensive psychosocial assessments
  • Develop individualized end-of-life care plans to meet patients’ psychosocial needs
  • Provide counselling, guidance, and advocacy for patients and families
  • Support patients and families in navigating community resources and services
  • Collaborate with interdisciplinary hospice teams to ensure holistic care
  • Participate in outreach activities, including community partnerships and family support programs
  • Liaising and assisting the Manager, Volunteer Services, as required

Qualifications:

  • Master of Social Work (MSW) from an accredited program
  • Member in good standing with the Ontario College of Social Workers and Social Service Workers
  • Recent experience in a healthcare setting, preferably palliative or hospice care
  • Strong interpersonal, organizational, and communication skills
  • Reliable, flexible, and self-directed professional, able to work collaboratively with a high-performance interdisciplinary team
