Cota is an accredited not-for-profit organization that has been providing community-based support to people living with mental health and cognitive challenges for over 50 years. At present, our wide array of services delivers much needed support to over 4,000 people/year who live with mental health challenges, psychogeriatric mental health conditions, acquired brain injuries, developmental disabilities, dual diagnoses and/or the experience of homelessness. These services include, but are not limited to, case management, supportive housing and adult day program services. Role Overview
As the Referral and Placement Coordinator for Cota's Transitional Age Youth (TAY) Program, your role is integral to the effective functioning of the program, ensuring a seamless transition for service users referred by child welfare agencies. You will not only oversee the critical administrative aspects of referral processing, budget negotiation, and placement coordination but also play a key role in fostering a supportive and culturally sensitive environment for service users. Your dedication to adhering to agency policies, promoting person-centered services, and maintaining strong communication channels will contribute significantly to the success of the program.
About the Team
The Referral and Placement Coordinator work alongside the TAY Team; including Managers, Case Managers, and Cota Finance Team. This program supports adults with developmental disabilities who were formerly in the care of one of the Toronto-based Children’s Aid Societies. The program ensures that clients receive quality support while they reside in privately operated fee-for-service residential care settings.
Primary Responsibilities:
- Oversee the administrative processing of incoming referrals to Cota's TAY Program from child welfare agencies
- Assign new clients to case management team
- Coordinate placement into appropriate based on client needs, risk factors, and program capacity
- Facilitate intake planning including transition timelines and required supports
- Monitor placement stability and address emerging issues in collaboration with program teams
- Collaborate in negotiating budget proposals for new service users, utilizing a standardized framework with defined budget ranges
- Maintain accurate and up-to-date referral tracking systems and documentation
- Engage in budget negotiations with external paid resources for current service users
- Ensure all clinical information is provided and assess service users’ clinical information to determine the most suitable budget considerations
- Collaborate with the administrative assistant to facilitate the submission of all new contractual agreements through the DocuSign platform
- Provide assistance in clarifying roles, handling financial inquiries, ensuring adherence to agency policies, and facilitating service agreement discussions
- Collaborate with the administrative assistant to facilitate the completion of onboarding processes for new agencies
- Generate and submit regular forecasting reports to the Ministry of Children, Community, and Social Services (MCCSS)
- Update/submit MYSLPs (expand the acronym) to MCCSS
- Provide assistance to case managers in handling emergency placement admissions
- Regularly liaise with external paid resources to assess placement availability
- Emphasize attention to cultural, racial, and identity considerations throughout the placement planning process to meet the needs of the service user
- Sustain consistent communication and liaison efforts with child welfare agencies and Cota’s management team
Other Responsibilities:
- Adheres to all Cota’s policies and procedures and follows established applicable program guidelines, procedures and practices
- Adheres to all applicable legislation, in particular, the Ontario Human Rights Code
- Complies with all Health and Safety policies and procedures to ensure the protection of workers and clients and that health and safety concerns or incidents are responded to promptly
- Works cooperatively, respectfully and constructively with clients, team members, management, other service providers, and external stakeholders
- Ensures that confidentiality of clients, employees and Cota information is maintained at all times
- Undertakes other related and/or delegated duties, as assigned
Qualifications:
Education:
- A post-secondary degree/diploma in Social Services, Social Work, or a related field (or equivalent experience)
Experience:
- Minimum of 2-3 years of work experience specifically and primarily with adults with developmental disabilities
- Previous training or work experience within the mental health sector and/or the justice system would be considered assets
- Experience working with external organizations and building positive working relationship
Knowledge & Skills:
- Expertise in trauma-informed clinical knowledge
- Familiarity with financial reporting
- Demonstrated proficiency in strong organization and administrative skills. The ideal candidate will possess the ability to effectively manage and prioritize tasks, ensuring efficiency and accuracy in day-to-day operations
- Strong clinical skills to accurately analyze and interpret clinical data
- Knowledge of and commitment to person-centered services
- Proven ability to work within a team, with partnership stakeholders and independently
- Strong professionalism with the ability to function as a representative of Cota in the community
- Excellent communication (verbal and written) and interpersonal skills
- Strong knowledge of Microsoft Office Suite (Word, Excel, etc.)
Other:
- The majority of working hours fall within regular business hours (Monday to Friday). Flexibility is required to ensure reports are generated in accordance with due dates
- Openness to discuss a hybrid/work from home arrangement is essential. However, it is not required to report to an office on a daily basis
