Job Description
Key Responsibilities:
- Provides comprehensive primary care to residents.
- Is knowledgeable of the Resident Bill of Rights and endeavors to respect and promote it.
- Conducts person-centered health assessment/physical of residents on admission with
specified time frame.
- Comply with applicable provincial legislation pertaining to long-term care, Long-Term
Care Homes Act 2007 & Regulation 79/10
- Participate in regular focused resident and family care conferences.
- Manages the care of residents by providing pharmacological, complementary and/or
counseling interventions.
- Provides outreach and transitional services to LTCH residents who return to the
community.
- Collaborates and consults with physicians, director of nursing and personal care nursing
staff, interdisciplinary team and external resources regarding resident plan of care.
- Provides leadership and involves the interdisciplinary team in quality improvement
initiatives.
- Provides formal and informal teaching and coaching in the management of clinical care.
Qualifications:
- Master’s of Nursing or equivalent with a clinical focus
- Completion of a Post-Master of Nursing (NP Field) Diploma
- Registration with the College of Nurses of Ontario, preferably within the RN (Extended
Class)
- 3 - 5 years of clinical nursing experience, preferably in Long-term Care
- Innovative critical thinking with excellent communication, public speaking and problem
solving skills.
- Knowledge of theoretical frameworks and research application
- Seeks out opportunities to maintain competency and continued learning.
- Maturity and good physical and mental health, empathy, patience and tact are important
qualities to possess.
