Position Overview:
Under the direction of the Access & District Mental Health Services Manager, the ICD and CMH Case Manager is responsible for the continuous implementation of the case management system. This includes ensuring regular, appropriate documentation on all clinical files; screening and developmental assessments for clients; regular conferencing/reviews of relevant cases; evaluating system effectiveness and recommending necessary changes; ensuring the current developmental, treatment, and psycho-social needs of clients are met by participating in the provision of assessment and counselling. A primary responsibility is to ensure the provision of case management and early intervention services for Children’s Mental Health Services that is consistent with the philosophy, policies, and standards established by Dilico Anishinabek Family Care.
Qualifications and Requirements:
- A Social Services degree or related diploma may be considered if complimented with clinical experience in children’s mental health focusing on ages 0-6.
- experience working with Anishinabek children, families andcommunities;
- a minimum three (3) years mental health case management experience with clients;
- a minimum three (3) years experience providing individual, family and group counselling, preferably with Anishinabek clients.
- candidates without the specific education qualifications and where the position does not require certified qualification to practice, but who possess an appropriate combination of experience and other academic qualifications will also be considered.
- Experience in developmental assessments and screening tools
- satisfactory Criminal Records Search;
- ability to work flexible hours as determined by the Counselling and Clinical Services Manager in relation to service requirements;
- must posses a Class “G” Driver’s License and have access to a personal vehicle;
- a valid Standard First Aid/CPR Certificate is recommended.
- must complete and maintain certification in CANS and other relevant Ministry of Children and Youth Services Initiatives.
Hours of Work: 32.5 weekly
