DESCRIPTION:
As a member of the Clinical Support Team at KidsInclusive, the Service Planning Coordinator will support families of children and youth with multiple and/or complex special needs by acting as the identifiable point of contact and being responsible for developing a coordinated service plan that recognizes all of their service needs and builds on their child/youth’s strengths. The Service Planning Coordinator will help families connect with the right services in their service delivery area and monitor how children and youth are progressing through the service plan as they grow. The Service Planning Coordinator provides direct and indirect client care; assists in providing departmental support; documents his/her involvement in client care; maintains communication with all other staff members; supports departmental efforts in Continuous Quality Improvement (CQI) activities; participates in education orientation and public relations, as required; maintains a safe work environment; performs other related duties as assigned.

Employees will demonstrate dignity and respect by being courteous and introducing themselves by name, occupation and role. Wherever possible, the Service Planning Coordinator will ensure that clients and families receive complete and accurate information and have the opportunity to ask questions. They will encourage clients and families to participate in care and decision-making at the level they choose, and collaborate by involving the client and family in the formulation of a plan of care.
DUTIES AND RESPONSIBILITIES:
Clinical:

Obtain client consent for assessment and/or intervention
-
Develop a strengths-based Single Plan of Care driven by the goals of the family, that addresses the service needs of the child/youth and is driven by the goals, vision and concerns of the family/child/youth;
-
Facilitate the active participation of the family in their child/youth’s Single Plan of Care;
-
Facilitate the exchange of information between relevant providers in the children’s services, education, and health sectors in each service delivery area, to develop and maintain a Single Plan of Care for the child/youth and their family.

Connect families to relevant services;
-
Explore flexible and innovative approaches for service delivery to meet the needs of the child/youth;
-
Monitor, review, and update the Single Plan of Care, in collaboration with the family and relevant providers in the children’s services, education, and health sectors, as the child and family’s needs and services change; and
-
Be knowledgeable and available to discuss the family’s concerns, if applicable, regarding their Single Plan of Care.

Administrative:

Maintain a record of all daily activities in AlayaCare as per KidsInclusive policy

Ensure maintenance of all client information, in accordance with privacy legislation and KHSC/KidsInclusive policies.

Participate in team meetings, discipline meetings and KidsInclusive meetings
-
Provide mentoring, student supervision and shadowing opportunities as requested by management
-
Provide input to the development of department and or hospital quality improvement procedures as requested.

Facilitate working relationships with providers in the children’s services, health and educations sectors, in order to enable their regular contribution into coordinated service planning and obtain and share relevant information regarding services for the child/youth.

Comply with relevant statistical, clinical and administrative reporting requirements

Comply with KHSC and KidsInclusive policies and procedures

Carries out duties in a way that embodies the mission and values of KidsInclusive and KHSC and works co-operatively with fellow employees, supervisors and departments toward the achievement of the organization’s overall goals and objectives

Assumes other duties that are assigned in accordance with experience, training and qualifications to which legislated requirements may be applicable
Educational:

Maintain a standard of professional education by attending educational programs and workshops; transmit information obtained from these workshops to fellow employees.

Complete e-learning modules as required by KHSC/KidsInclusive policies

Assume educational responsibilities as required by KidsInclusive

Maintain a record of all educational activities on AlayaCare

MINIMUM REQUIREMENTS:

A diploma or degree in child /youth/family studies or social services or equivalent combination of education and experience
-
Three years’ experience working in the children’s service sector, preferably with children with special needs and their families
-
Proficiency in speaking and comprehending both English and French preferred
-
Experience with and knowledge of behavioural concepts and general counselling skills
-
Experience working with multiple community agencies that serve children and youth
-
Excellent communication skills and proven ability to work as part of a team with professionals, support staff and other community agencies
-
Demonstrated good organizational skills and reporting skills and has a strong ability to multi-task
-
Access to a reliable vehicle along with a valid driver's license and automobile insurance as per hospital policy
-
BCLS at the Health Care Provider level
-
Proven ability to attend work regularly.

Satisfactory Criminal Reference Check required
