Job Summary: In accordance with the Mission, Vision and Values, and strategic directions of Provincial Health Services Authority patient safety is a priority and a responsibility shared by everyone at PHSA, and as such, the requirement to continuously improve quality and safety is inherent in all aspects of this position. Reporting to the Program Manager and Medical Director Oncology/Hematology/BMT Program, the Quality & Safety Leader, Oncology/Hematology/BMT works in collaboration with the Quality Safety and Risk Management Department to promote application and integration of evidence-based practice to increase the effectiveness, efficiency, accountability and sustainability of clinical activities within the Oncology/Hematology/BMT program and at the Children’s & Women’s Health Centre (C&W). In addition, spread of unit based initiatives will be applied to other agencies within and external to the Provincial Health Services Authority (PHSA). The Leader plays an integral role by providing leadership and expertise in the application of quality improvement, patient safety, and risk management principles and practices to activities. In accordance with long- and short-term C&W and PHSA strategic plans and Canadian Council on Health Services Accreditation (CCHSA) accreditation goals and standards, the Leader defines, facilitates, manages, and completes a wide variety of quality improvement initiatives and activities by providing initiative, leadership, support and direction to, and collaborating with personnel throughout the C&W. Using established quality tools and methods, the Leader provides leadership on quality initiatives to staff within C&W programs and departments. The Quality Leader provides leadership in accreditation activities for the BMT component of the program, working collaboratively with the Director, BMT and appropriate program, department, division, hospital, and PHSA staff and physicians. The Quality Leader, in accordance with the FACT (Foundation for the Accreditation of Cellular Therapy) standards, identifies quality objectives and strategies to improve clinical outcomes, patient/client access and safety and provides leadership through the development of quality management plans and policies. Key responsibilities include: quality project management, incident report management, quality indicator development, monitoring and reporting, benchmarking and process measurement, patient safety promotion, developing and supporting the delivery of education programs related to quality and mentoring Oncology/Hematology/BMT staff within the quality improvement framework. These responsibilities are guided by the Achieving Improved Measurement (AIM) quality framework used by the CCHSA in consultation with other key quality and safety requirements and the FACT standards and guidelines and FACT Quality Management Plan. Duties/Accountabilities: Scope and Complexity Working collaboratively with Oncology/Hematology/BMT program leaders and C&W leaders, the Department of QS&RM, the Department of Learning and Development, and Senior Administration, the Quality & Safety Leader identifies objectives and strategies to improve quality in all dimensions including safety. The Leader provides leadership by identifying risks and engaging and building improvement teams utilizing quality improvement expertise in the planning, implementation, and evaluation of initiatives based upon best evidence to improve quality of care, safety, and risk reduction. The Leader contributes to the development of the Oncology/Hematology/BMT program budget related to cost reduction associated with quality improvement initiatives. The Leader participates actively in strategic planning and defining goals and objectives for Oncology/hematology/BMT, and offers coaching and mentoring to program staff, and C&W front-line leaders. The Leader utilizes considerable creativity and problem-solving skills to develop and implement effective policies and processes for quality and safety, consistent with PHSA policies and processes. The ability to foster change throughout a complex organization is important, and consensus-building is key to the success of this role. The ability to communicate clearly and effectively with personnel within the program and across the organization regarding quality and safety issues and initiatives is critical. Decision making in this role affects a wide range of activities and goals which are of considerable importance to patients, professional staff and the organization. The issues and functional responsibilities for this role are diverse and evolve in response to external and internal influences, legislative changes, accreditation requirements, and strategic priorities. Many of the issues that arise are unique. The ability to problem-solve and exercise independent judgment in reaching decisions is required, and the ability to determine when consultation with leaders or external advisors is necessary is essential. Communications Internally, the Quality & Safety Leader works closely with people at all levels within C&W to build relationships and knowledge sharing capacity, and ensures that personnel are informed of quality activities/projects that will have an impact on their responsibilities. Contact with the Senior Director Operations, Pediatric Specialty Medicine and program leaders and other professionals primarily involves negotiating and/or facilitating consensus for quality initiatives at the leadership level of the committee, program, department or service. The Leader works with a primary focus within the Oncology/Hematology/BMT Program and with staff from the clinical programs and various departments on quality initiatives and projects. The Leader collaborates with representatives from Nursing, Medicine, Allied Health, Decision Support Services, Laboratory and other stakeholder groups to build and implement an integrated quality program within Pediatric Oncology/Hematology/BMT and partnership programs, and works as an essential member of the Pediatric Oncology/Hematology/BMT team to ensure ongoing alignment with overall organizational and corporate strategic directions. Externally, the Leader consults and collaborates with counterparts at other health care organizations regarding common issues; professional organizations, such as the colleges and registration bodies, regarding quality requirements and issues, legal advisors and external experts on quality and safety issues as needed. Duties/Accountabilities 1. Provides leadership and quality improvement expertise in the planning, implementation, and evaluation of initiatives designed to improve quality of care, safety, and risk reduction in Pediatric Oncology/Hematology/BMT. 2. Monitors Pediatric Oncology/Hematology/BMT staff and physician perceptions of safety and quality and identifies learning opportunities related to the integration of quality improvement methodology and evidence-based systems and tools with everyday work processes through regular contact with clinical and service providers, visits to the clinical areas, membership on Quality of Care Committee, and participation in key organizational change initiatives. Develops, implements, and evaluates education plans to address identified learning needs. 3. Provides consultation, guidance, and support to Pediatric Oncology/Hematology/BMT and other staff, teams, and committees in the use of tools and mechanisms to measure, monitor, and evaluate initiatives designed to improve patient care outcomes. 4. Provides facilitation, coaching, and support to improvement teams in the development, monitoring, and analysis of key performance indicators, in alignment with other levels of reporting within C&W and the PHSA. 5. Provides expert support for analysis of multiple sources of data to promote a balanced approach to identifying and acting upon improvement opportunities. 6. Provides leadership in the development and implementation of patient safety and risk indicators, alert systems and other risk prevention strategies. 7. Evaluates the effectiveness of the application of C&W processes as they relate to patient safety, complaints management, and incident reporting. Makes recommendations based on evaluation of the impact to patient outcomes and organizational risk. 8. Manages Pediatric Oncology/Hematology/BMT incident reports using an intake and follow up system that ensures reports and follow up are completed in a timely manner, thoroughly reviewed, and processed in keeping with a non-punitive reporting culture, and that all resulting recommendations are implemented. Identifies trends, makes recommendations based on the trends, and formally reports these trends to the program leaders, Quality of Care Committee and others as appropriate. Reports actual or potential risks to patient outcomes and/or to C&W to the Senior Director of Operations, Pediatric Specialty Medicine, Pediatric Oncology/Hematology/BMT Program Leaders, and C&W Director, QS&RM for follow up. 9. Contributes to the development of C&W policies related to patient safety, risk management, and applicable legislation and provides leadership in implementation of and compliance with same. 10. Performs a leadership role in the integration of accreditation principles and standards with C&W initiatives. Working collaboratively with PHSA and C&W accreditation leaders, coaches and facilitates teams participating in accreditation self-assessment, planning, and survey preparation and follow up. 11. Provides consultation and support for development, implementation and evaluation of actions identified through self-assessments and in response to recommendations from the CCHSA and other external bodies directly applicable to Pediatric Oncology/Hematology/BMT and C&W as requested. 12. Provides expertise and facilitation in implementing and evaluating evidence-based clinical decision support tools such as protocols, care paths, structured improvement collaboratives, the Plan-Do-Study-Act (PDSA) cycle (Model for Improvement), Root Cause Analysis (RCA) and Failure Mode and Effects Analysis (FMEA). 13. As a member of the Pediatric Oncology/Hematology/BMT Quality Assurance Committee and participant in C&W quality review activities, provides expertise to the quality review process, identification of recommendations, follow up action, and reporting of improvements and monitors, investigates and responds to system issues arising from quality reviews, reports (including Coroner’s reports), and alerts (such as Health Canada alerts). Ensures that reviews undertaken are complete and recommendations are acted upon. Facilitates the implementation of approved initiatives, and monitors and follows up as required. 14. Liaises with external supports and resources to provide specialized knowledge, share information, and collect data. 15. Provides leadership within the Pediatric Oncology/Hematology/BMT program in interactions with groups and individuals at the C&W through coaching, guiding, and modelling key behaviours/strategies, encouraging dialogue, and providing guidance and advice to facilitate resolutions to work issues. Assists others to define shared and individual goals, meet target dates, and ensure alignment of goals with customer needs and the goals of senior leaders. Fosters a team spirit, trust and mutual respect. 16. Identifies own learning needs that contribute to skill enhancement and career progression for self, and fosters a working environment that stimulates learning and inquiry for self and others. Takes responsibility to be current about new initiatives, developments and trends in quality through literature review, contact with peers inside and outside the C&W, industry advisor groups, professional associations, and attendance at seminars, workshops and other educational programs. Communicates information and makes recommendations to the C&W Leaders and QS&RM for evaluation and approval. 17. Evaluates own performance annually using PHSA processes, and contributes to the evaluation of team performance in collaboration with appropriate internal stakeholders and peer staff. 18. Performs other related duties as assigned. Qualifications: A level of education, training and experience equivalent to an undergraduate degree in a health discipline and Master’s preparation in Nursing, Leadership, Health Administration, Business or a related health discipline, plus a minimum of five (5) years of clinical practice and professional management/leadership experience. Previous experience and education in quality, patient safety, risk management or accreditation is preferred. A clinical background in Pediatric Oncology/Hematology/BMT is preferred. In addition, the Quality & Safety Leader must demonstrate leadership skills, specifically: Leading People by acting with integrity and trust, building a team environment, communicating clearly, and fostering potential; Leading Change by striving for personal mastery, leading with vision, inspiring and engaging others, and promoting innovation; Partnering by focusing on customers and engaging stakeholders; Achieving Results by conceptualizing and being accountable; and Business Acumen in managing resources and showing health environment and political awareness. The Leader requires a working knowledge of specific federal and provincial legislation. Demonstrated analytical skills.
