Occupational Therapist

About the role

The Occupational Therapist brings a functional and environmental lens to crisis intervention, supporting immediate safety, independence, and stabilization while reducing risk and avoiding unnecessary hospital admissions or calls to emergency services. DUTIES AND RESPONSIBILITIES · Respond to community-based crisis situations as part of a 24/7 mobile response team · Complete rapid functional, cognitive, and environmental assessments in dynamic environments · Identify immediate safety risks related to mobility, cognition, ADLs, and living conditions · Develop and implement short-term, goal-oriented intervention plans focused on stabilization and safety · Utilize trauma-informed, client-centered approaches to support de-escalation and engagement · Provide education and strategies to clients and caregivers to promote independence and reduce risk · Recommend and facilitate access to assistive devices, environmental modifications, and community supports · Collaborate with interdisciplinary team members, community agencies, hospitals, and other stakeholders · Support transitions of care and appropriate referrals to community resources and ongoing services · Maintain accurate and timely documentation in accordance with College standards and Bayshore policies · Participate in quality improvement and program development initiatives · Complete other tasks as required

Education · Degree in Occupational Therapy from a recognized institution · Registered and in good standing with the College of Occupational Therapists of Nova Scotia Experience · Minimum of two (2) years of community-based or relevant clinical experience (preferred) · Experience in one or more of the following areas:

  • Mental health and crisis intervention
  • Community or home care
  • Functional/ADL assessments
  • Environmental or home safety assessments
  • Experience working with individuals experiencing complex psychosocial needs is an asset SKILLS AND COMPETENCIES · Strong clinical reasoning and decision-making skills in unpredictable environments · Ability to de-escalate crisis situations and adapt interventions in real time · Excellent communication, documentation, and interpersonal skills · Ability to work independently and collaboratively within a team · Strong organizational and time management skills · Commitment to client-centered, trauma-informed care REQUIREMENTS · Valid driver’s license and reliable vehicle · CPR Certification (CPR-C or BLS) · Vulnerable Sector Check · Professional liability insurance · Access to a secure computer and internetave.
