ABOUT THE ROLE
The Chronic Pain Program requires a highly organized team player who will join our interprofessional team to assist us in carrying out the mission of St. Joseph's Care Group. The occupational therapist role requires knowledge and related experience with triage and decision guide processes functional assessments mental health concerns and the complexity of chronic pain. It also requires an appreciation of accessibility accountability best practices and evaluation. The chronic pain program provides interprofessional holistic programming that includes people-centred strength based care and self management principles.

WHAT YOU BRING
Education/Experience:
-
Current registration or eligibility for registration with the College of Occupational Therapists of Ontario
-
Experience in providing individual and group interventions psychoeducation and public education
-
Demonstrated experience working alone in the community with adherence to safety and working-alone procedures
-
Experience working within an interprofessional team setting with the ability to work collaboratively as a team player with excellent interpersonal skills excellent judgment critical thinking skills problem-solving and time management skills
Skills/Abilities:
-
Knowledge and experience with a Decision Guide model of care triage of referrals motivational interviewing and strengths-based approaches to care psychosocial rehabilitation and the principles of chronic disease self-management
-
Knowledge and experience working with an adult population with serious and persistent mental health concerns and the development execution and evaluation of treatment plans utilizing SMART goals
-
Excellent knowledge of internal and external community resources
-
Clear and effective communication skills in oral written and electronic communications
-
Understanding of relevant legislation (Mental Health Act Health Care Consent Act and PHIPA)
-
Demonstrated regular attendance at work and adherence to the SJCG Principles of Conduct
Conditions of Employment:

This position shall be conditional upon a satisfactory Police Vulnerable Sector Check (PVSC) where indicated, to ensure the absence of relevant criminal convictions.

Offers of employment made to external candidates are conditional upon providing proof of a complete primary series of Health Canada approved mRNA COVID-19 vaccine or proof of valid medical exemption, prior to the offered effective date.
