About the role
Reporting to the Manager, Access & Flow, the Patient Flow Specialist will be responsible for the coordination of activities to enhance patient flow and facilitate patient placement throughout the Health Centre.
Accountabilities:
Putting patients first
- Accountable for patient placement throughout the organization and has the authority in determining bed assignment and provides assistance for individuals assigning beds.
- Facilitates daily bed management meetings, advises stakeholders of overall corporate bed status, and works with interprofessional team to follow up on issues or barriers to flow throughout the day
- Collaborates and liaises between all patient care services, medical staff, bed management, and hospital support programs and services, to ensure that patients are placed in the most appropriate available bed the first time.
- Assists with coordination of internal transfers of patients between units and transfers of external patients between facilities.
Enhance the health of the communities we serve
- Develops and nurtures partnerships with internal and external communities
Create a culture of inquiry and innovation
- Data analysis of quality, utilization and outcome indicators, developing and implementing innovative plans to achieve required targets for enabling Access & Flow.
- Reviews and simplifies systems/structures/policies to support the vision of the program/Health Centre
- Identify opportunities for patient flow improvements and act as a subject matter expert for patient flow projects and initiatives.
Use resources wisely
- Reliable and flexible individual to work as a part of a diverse work team. Must be available to work day, evening, weekend rotation.
Inspire our people
- Provides leadership, direct supervision for bed booking staff while on shift
- Facilitates and supports effective team behaviour/development of a high performance team
Qualifications:
- Bachelor's degree in a health-related discipline required; Master's an asset
- Registration in good standing with relevant professional college or regulatory body in Ontario.
- Minimum 3-5 years recent acute care experience
- Excellent conflict resolution skills to investigate and resolve problems.
- Demonstrated evidence of innovative leadership and critical thinking, with proven ability to foster a collaborative team approach to decision making
- Demonstrated ability in communication, team building, problem solving and change management within an interprofessional environment
- Strong understanding of bed management/utilization and patient flow methodologies as they relate throughout the continuum of care
- Understanding of continuous quality improvement tools and methodologies
- Intermediate level computer skills (MS Office, email, electronic health record system.) Experience with bed management software an asset
- Evidence of continuing professional/personal development
- Demonstrated excellent communication skills including written, verbal and English comprehension
- Satisfactory attendance record.
