Right at Home Canada Fraser Valley is part of an international family of over 700 Care Offices around the globe. We provide a full suite of both non-medical and medical care, products and services to older adults and disabled adults who want to continue to live independently, wherever home is for them.
Right at Home In-Home Health is currently seeking a part time Care Planner to become part of our team in our Abbotsford office. This is a 25-30 hrs/wk position.
The Care Planner is responsible for incoming inquiries, client assessments, creating the client care plan, and quality assurance. This position will also handle staff training, staff orientation, scheduling staff and maintains scheduling information and data as required. Our company is growth focused and looking for a passionate individual who takes initiative, looks at challenges as opportunities and always follows through.
DUTIES AND RESPONSIBILITIES

Respond and document incoming referral/inquires regarding services.

Meet with potential clients and family members to assess their needs and provide solutions in the form of a care plan.

Ensure safe, appropriate and meaningful care is being provided to client and families.

Possess the ability to plan and create care teams to support client care needs.

Ability to plan and implement weekly and monthly quality control visits and provide client/staff supports as needed.

Assist in scheduling employees for care, based on the match between the client’s needs and the right employees.

Serves as a mentor to caregivers and support their care delivery.

Onboarding new staff and ensure all relevant documentation is received.

Schedule and conduct caregiver orientation and set up training needs.

Promote Right at Home supportive care services in the community.
Education/Experience Requirements:

Completion of a recognized Program, such as LPN, OT/PT; an equivalent work experience may be considered.

Two years experience in home care, health care or senior-related industry required
-
Proficiency with computers, learning new software (Alayacare/Inflowcare) and Microsoft Office (Outlook, Word, Excel).
Other Skills and Abilities

You possess excellent interpersonal and communication skills (proficiency in both verbal and written English).

You have the ability to multitask, prioritize, and exhibit time management in a fast-paced environment.

Professionalism is a must, with close attention to detail in task completion.

You have the ability to have difficult conversations.
